The site has historically been used as a municipal landfill and as a green waste repository. Exhibit 1 (Appendix A) is a site diagram indicating the approximate locations of the site investigation areas and sample locations in relation to pertinent site features. 1.2 Project Information TOSA is interested in developing the eastern portion of the site with tiny homes, several larger community buildings, open air pavilions, roadways and a parking lot. The west side of the property may be used at a later date for outdoor uses, such as a park, playgrounds or gardening areas, if determined to be suitable for such uses. In addition, TOSA is interested in learning if the soil placed on top of the landfill debris on the west side of the site would be suitable for use as fill in other parts of the site, specifically the low area on the east side of the property. An LSI conducted by Terracon (report dated July 1, 2021, Terracon Project No. 61217255 Task 1) attempted to better delineate the nature and extent of landfill debris on the eastern portion of the site. Observations made during the excavation of 17 test pits at the site indicate approximately one third of the site has received little or no landfill debris. The remaining areas are covered either by the large landfill pile or fill ranging from 2.5 to 7 feet thick. While measurements made at the site with direct-reading instruments did not suggest landfill gases are being generated in significant amounts, Terracon recommended soil gas samples be collected and analyzed for methane and volatile organic compounds (VOCs). Additionally, a drainage ditch running through the approximate center of the site was observed to be stained with possible petroleum hydrocarbon or other suspect deposits. Terracon recommended collecting samples along the ditch to investigate the possible presence of contaminants in sediments and shallow soils within the ditch. Supplemental Limited Site Investigation Proposed TOSA Village on Indiana Avenue ■Salt Lake City, Utah August 26, 2021■Terracon Project No. 61217255_2 Responsive ■Resourceful ■Reliable 2 1.3 Objectives The Supplemental Limited Site Investigation (LSI) was designed to evaluate the presence of VOCs and methane in soil gas, with an emphasis on areas where larger community buildings would be sited, and the presence of typical industrial contaminants in sediments and shallow soils along the ditch in areas where surficial staining has been observed. 1.4 Standard of Care Terracon’s services were performed in a manner consistent with generally accepted practices of the profession undertaken in similar studies in the same geographical area during the same time. Terracon makes no warranties, express or implied, regarding the findings, conclusions, or recommendations. Terracon does not warrant the work of laboratories, regulatory agencies, or other third parties supplying information used in the preparation of the report. These LSI services were performed in accordance with the scope of work agreed upon with you, our client, as reflected in our proposal and were not restricted by ASTM E1903-19. 1.5 Additional Scope Limitations Findings, conclusions, and recommendations resulting from these services are based upon information derived from the onsite activities and other services performed under this scope of work. Such information is subject to change over time. Certain indicators of the presence of hazardous substances, petroleum products, or other constituents may have been latent, inaccessible, unobservable, non-detectable, or not present during these services, and we cannot represent that the site contains no hazardous substances, toxic materials, petroleum products, or other latent conditions beyond those identified during our investigation. Subsurface conditions may vary from those encountered at specific borings or wells or during other surveys, tests, assessments, investigations or exploratory services; the data, interpretations, findings, and our recommendations are based solely upon data obtained at the time and within the scope of these services. 1.6 Reliance This report has been prepared for the exclusive use and reliance of The Other Side Academy (client). Use or reliance by any other party is prohibited without the written authorization of The Other Side Academy and Terracon. Supplemental Limited Site Investigation Proposed TOSA Village on Indiana Avenue ■Salt Lake City, Utah August 26, 2021■Terracon Project No. 61217255_2 Responsive ■Resourceful ■Reliable 3 2.0 LIMITED SITE INVESTIGATION METHODOLOGIES 2.1 Utility Clearance Prior to conducting the subsurface investigation, the public utility locating service (Blue Stakes) was notified to arrange for underground utility locates at the above-referenced site. 2.2 Soil Gas Samples Soil gas samples were collected August 4, 2021. Prior to sampling, five soil gas implants (SG-1 through SG-5) were installed at the site on July 30, 2021. Three of the implants were positioned toward the southern end of the property in areas where the proposed development includes community buildings (SG-1, SG-2, and SG-3). The two remaining implants were positioned centrally and toward the north end where general residential structures are planned (SG-4 and SG-5). Soil gas sample locations are shown on the Site Diagram Exhibit 1 (Appendix A). The soil gas implants were installed using a direct-push drill rig to advance a boring to an approximate depth of 4 to 6 feet bgs. The implants consist of a 6-inch long stainless steel disposable screen anchored to a polyethylene tubing that extends to the surface. A one-foot sand pack, created of 10-20 silica sand, was placed around the implant and the remainder of the anulus was filled with hydrated bentonite chips. Care was taken to ensure adequate hydration of the chips to avoid air flow through the bentonite which might dilute the soil gas sample. The soil gas implants were allowed to equilibrate for twenty-four hours before sampling. In addition, due to heavy rains following the installation of the implants, sampling was delayed three additional days to ensure soils were not saturated. Prior to sample collection, Terracon conducted leak detection tests at each sample location. The first test was to evaluate the sample train for leaks by conducting a shut-in test on the sample train after it was fully assembled. The shut-in test was conducted by closing the valve to the vapor point and applying a vacuum to the closed sample train while monitoring the pressure on an inline vacuum gauge for one minute. If the sample train held a constant vacuum pressure, the sample train was deemed to be airtight. Following the shut-in test, the air that was evacuated from the sample train to generate the vacuum was purged. A second leak detection test was conducted by introducing helium gas at a concentration between 15 and 20 percent into a sampling shroud placed over the probe. If helium was measured in the sampling train at a concentration of greater than 7,500 parts per million (ppm), it indicated there was potential breakthrough in the sampling train and all connections were re-examined and tightened where necessary and re-tested. Three helium tests were conducted at each location prior to sampling. Supplemental Limited Site Investigation Proposed TOSA Village on Indiana Avenue ■Salt Lake City, Utah August 26, 2021■Terracon Project No. 61217255_2 Responsive ■Resourceful ■Reliable 4 The five soil gas samples were collected in laboratory-supplied, batch certified clean 400-mL Summa® canisters, using new Teflon® tubing and a flow regulator that restricts flow to no more 200 mL per minute. 2.3 Shallow Soil Samples in North Ditch Shallow soil samples along the North Ditch were collected on July 30, 2021. Sample locations were selected based on the presence of staining and on topographically low points along the ditch where runoff and/or discharge would be expected to pond. The shallow soil sample locations are shown on the Site Diagram (Exhibit 1, Appendix A). Shallow soil samples were collected from the sediments and surficial soils from 0 to 4 inches below the surface using a hand trowel. Sampling equipment was cleaned using an Alconoxâ wash and potable water rinse before beginning the project and before collecting each soil sample. The shallow soil samples were placed in laboratory-prepared containers, labeled, and placed on ice in a cooler. The samples and completed chain-of-custody documentation were subsequently shipped via FedEx overnight to Pace National Analytical Laboratories, a Utah-certified laboratory, for analysis on a standard turnaround. 3.0 LABORATORY ANALYTICAL PROGRAM The soil gas samples were analyzed for volatile organic compounds (VOCs) using EPA Method TO-15 and Methane using EPA Method 8015M. The shallow soil samples were analyzed for constituents commonly associated with industrial uses: §Polycyclic aromatic hydrocarbons (PAHs) using EPA Method 8021 §Volatile Organic Compounds (VOCs) using EPA Method 8260; §Pesticides and PCBs, using EPA Methods 8081/8082; §RCRA 8 Metals (arsenic, barium, cadmium, chromium, lead, selenium, silver and mercury) using EPA Methods 6010 and 7000. 4.0 RESULTS Exhibit 1 (Appendix A) shows the locations of the shallow soil and soil gas samples as well as pertinent site features, specifically the North Ditch. 4.1 Soil Gas Sample Results The analytical results for VOCs and methane in soil gas are summarized in Table 1 (Appendix B). Only compounds which were detected in at least one sample are displayed in the table. The analytical results were compared to the U.S. EPA’s Target Sub-Slab and Near- source Soil Gas Concentration Vapor Intrusion Screening Levels (VISLs) for residential and commercial scenarios. The VISLs use a Target Cancer Risk of 1x10-6 (one-in-a-million) and a Hazard Quotient of 1. As shown in Table 1 (Appendix B), several VOC compounds were detected in the five samples. Benzene exceeded the residential VISL in the sample from SG-3 and chloroform exceeded the residential VISL in the sample from SG-4. No sample exceeded a commercial VISL. EPA has not established a VISL for methane; however, the Lower Explosive Limit (LEL) is used to evaluate the physical hazard posed by methane, due to its explosive characteristic. The LEL for methane is 5 percent which is equivalent to 50,000 parts per million by volume (ppmv). The soil gas sample from SG-3 reported a methane concentration of 23,000 ppmv which is approximately one half of the LEL. 4.2 Soil Sample Results The analytical results for the three North Ditch surface soil samples are summarized in Table 2 (Appendix B). Only compounds which were detected in at least one sample are displayed in the table. The laboratory analytical report and chain of custody are presented in Appendix C. The analytical results were compared to the U.S. Environmental Protection Agency’s (EPA’s) Regional Screening Levels (RSLs) for soil under both residential and commercial scenarios. The RSLs use a Target Cancer Risk of 1x10-6 (one-in-a-million) and a Hazard Quotient of 1. As shown in Table 2, lead was reported at 548 milligrams per kilogram (mg/kg) in the sample ND- 03. This exceeds the RSL for residential properties of 400 mg/kg. The polycyclic aromatic hydrocarbon, benzo(a)pyrene was also reported above the residential RSL in the sample from ND-02. It was reported at 0.466 mg/kg compared to the residential RSL of 0.11 mg/kg. Neither of these compounds exceed the commercial RSL. Arsenic was reported in all three samples at concentrations ranging from 18.1 to 24.6 mg/kg. This exceeds both the residential and commercial VISLs of 0.68 and 3 mg/kg, respectively. 5.0 CONCLUSIONS AND RECOMMENDATIONS 5.1 Summary of Soil Sample Results Lead and arsenic were the only two metals analyzed that exceeded the U.S. EPA’s residential RSLs. Lead exceeding the residential RSL was present in one of the three soil samples. Although arsenic is also reported at concentrations that exceed the RSLs, the concentrations are consistent Supplemental Limited Site Investigation Proposed TOSA Village on Indiana Avenue ■Salt Lake City, Utah August 26, 2021■Terracon Project No. 61217255_2 Responsive ■Resourceful ■Reliable 6 with what is generally considered naturally occurring background levels for the Salt Lake Valley and is less than cleanup levels generally accepted by the DEQ. PAH-impacted soils exceeding the U.S. EPA’s residential RSLs were reported in one soil sample at a concentration that exceeds the residential RSL. PAHs and lead have not been observed widely across the site, suggesting these are limited occurrences. 5.2 Summary of Soil Gas Sample Results Several VOC compounds were reported in one or more of the three soil gas samples. Benzene was reported in the soil gas sample from SG-3 in the southern area of the site at a concentration that exceeds its residential VISL, and chloroform exceeded the residential VISL in the sample from SG-4. None of the other reported constituents exceeded the residential VISLs. The occurrence of low concentrations of two VOCs in two of the five soil gas samples suggest a vapor intrusion condition may exist at the site. One of the five soil gas samples reported an elevated concentration of methane (approximately one half of the LEL). This suggests a source of methane gas is present in this area, likely a deposit of landfill or green waste that is still decomposing. Based on the results of this Supplemental LSI, Terracon recommends that vapor mitigation be considered during development of larger buildings in the areas where VOCs exceeded VISLs. In addition, limited removal of landfill and green waste in the area of elevated methane gas should be performed. Finally, elevated concentrations of lead should be addressed through either capping or removing impacted soils. Terracon appreciates being of service to you on this project. This is not intended as a comprehensive explanation, and if you have additional questions please contact your project representative. Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name, Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received. Abbreviations and Definitions (dry)Results are reported based on the dry weight of the sample. [this will only be present on a dry report basis for soils]. MDL Method Detection Limit. MDL (dry)Method Detection Limit. RDL Reported Detection Limit. RDL (dry)Reported Detection Limit. Rec.Recovery. RPD Relative Percent Difference. SDG Sample Delivery Group. (S) Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media. U Not detected at the Reporting Limit (or MDL where applicable). Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes reported. Dilution If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the result reported has already been corrected for this factor. Limits These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or duplicated within these ranges. Original Sample The non-spiked sample in the prep batch used to determine the Relative Percent Difference (RPD) from a quality control sample. The Original Sample may not be included within the reported SDG. Qualifier This column provides a letter and/or number designation that corresponds to additional information concerning the result reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable. Result The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL” (Below Detectable Levels). The information in the results column should always be accompanied by either an MDL (Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect or report for this analyte. Uncertainty (Radiochemistry)Confidence level of 2 sigma. Case Narrative (Cn) A brief discussion about the included sample results, including a discussion of any non-conformances to protocol observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report. Quality Control Summary (Qc) This section of the report includes the results of the laboratory quality control analyses required by procedure or analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not being performed on your samples typically, but on laboratory generated material. Sample Chain of Custody (Sc) This is the document created in the field when your samples were initially collected. This is used to verify the time and date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the samples from the time of collection until delivery to the laboratory for analysis. Sample Results (Sr) This section of your report will provide the results of all testing performed on your samples. These results are provided by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for each sample will provide the name and method number for the analysis reported. Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and times of preparation and/or analysis. Qualifier Description B The same analyte is found in the associated blank. J The identification of the analyte is acceptable; the reported value is an estimate. J3 The associated batch QC was outside the established quality control range for precision. J4 The associated batch QC was outside the established quality control range for accuracy. J5 The sample matrix interfered with the ability to make any accurate determination; spike value is high. P RPD between the primary and confirmatory analysis exceeded 40%.
