As we discussed during our visit, you had concerns and questions about the prior sanitary survey completed on 8/29/2024. As a result of our site visit we can make the following corrections to the results of the sanitary survey: M003, M004, M005 can all be removed. M006 and M007 will remain until a log of applicable activities has been established. S015 - This deficiency can be removed for both WS001 and WS002. The other deficiencies on WS001 and WS002 will remain as we cannot verify that there is a check valve, meter or pressure gauge on either well. All of the deficiencies for ST001 can be removed as this tank has now been abandoned. V035, V009 can be removed, however V007 will remain on ST002 as gaps in the #14 mesh screen can be seen in the attached photo. T006 can be removed as the sample tap for finished water in the tap at each home. SIG - 50pts - SO - S013 - WELL LACKS THE REQUIRED WELL SEAL: R309-515-6(6)(i) - R309-515-6(6)(i) and R309-515-6(12)(b,c,d)require a sanitary seal be installed and maintained at the wellhead and discharge piping.  See attached photo.
