Table 1 presents information for each abandoned well including the location, known status of the well, well depth, screen interval, date of construction, construction materials, and type of surface completion. Figure 1 presents the locations of the abandoned wells. In a letter dated 13 December 2022, TEAD-N submitted a request to the Division of Waste Management and Radiation Control (DWMRC) to abandon 25 monitoring wells that were either obstructed or dry and no longer functional. The DWMRC granted the request in a letter dated 12 January 2023, with the requirement that a well abandonment work plan be submitted to and approved by the DWMRC (DWMRC 2023). The Dry/Obstructed Well Abandonment Work Plan (Brice Engineering, LLC [Brice] 2023) was submitted to the DWMRC on 11 September 2023 and approved on 6 October 2023. 2.0 PRE-MOBILIZATION Brice subcontracted Conetec, a Utah-licensed driller, to conduct the well abandonment. Prior to commencing well abandonment work, Conetec notified the state engineer of the proposed abandonment work in accordance with Utah Administrative Code (UAC) Subsections R655-4-4.2.6 and R655-4-14.2.1. 3.0 WELL ABANDONMENT PROCEDURES Monitoring wells were abandoned between 8 November 2023 and 12 January 2024. Well abandonment activities were performed in accordance with Subsection R655-4-14.3.1 of the UAC; all work was overseen by a Brice Geologist. Each well proposed for abandonment was pressure grouted, above ground completions were removed, and the well casing cut off 1 foot below ground surface. At locations where only one of a nested well pair (shallow and deep wells in same borehole and surface completion) was being abandoned, the abandoned well was pressure grouted and remains in place within the surface completion. Prior to grouting each well, a water level indicator was lowered to the bottom of the well to locate any obstructions and any downhole materials encountered, were removed. In wells with obstructions that could not be removed the driller attempted to clear the blockage with the tremie pipe. Table 1 presents well information including obstruction and total depths. Portland cement grout was mixed with potable water per manufacturer’s instructions and placed from the bottom of the well progressively upward using a tremie line and grout pump. During abandonment of monitoring wells, A-03 and P-04S, a large amount of grout was added with minimal upward filling of the well casing. The surrounding formation past the sand filter pack caused the grout to move horizontally instead of filling the vertical casing. When enough grout was in place to fill the well screen interval it was left to cure overnight. Once the grout was set, additional grout was added up to the natural ground surface. At all other monitoring wells grout was able to be emplaced in a single batch. Table 1 presents the approximate volume of grout required to abandon each well. Dry/Obstructed Well Abandonment Report 2 Environmental Remediation Tooele Army Depot North, Utah After grouting, the surface completions were demolished, which included the removal of the concrete pad, protective metal casing, and bollards. Demolition activities were conducted using a backhoe and acetylene torch, which was used to cut the metal casing. Prior to use of the torch, a Match, Flame, and Heat Producing Equipment Permit was obtained from TEAD-N. Following demolition, the area was regraded to match surrounding conditions using native soil. Appendix A presents a photograph log of well abandonment activities. Solid waste generated during well abandonment was disposed of in the municipal waste stream by the drilling contractor. 4.0 POST-ABANDONMENT In February 2024, Conetec filed the abandonment log with the State Engineer at Utah Division of Water Rights (Appendix B). 5.0 REFERENCES Brice Engineering, LLC (Brice).
