Dear Justun Dave Edwards:

Our records indicate that your system is in violation of the Environmental Protection Agency
(EPA) National Primary Drinking Water Regulation 40 CFR 141.90 (e). The initial lead service
line inventory was due October 16th, 2024.

This letter serves as a notice that the HERRIMAN CITY MUNICIPAL WATER DEPARTMENT
failed to complete and submit the initial lead service line inventory by the October 16th, 2024
deadline. Failure to complete and submit the initial lead service line inventory by the deadline
may lead to the Environmental Protection Agency (EPA) taking further enforcement action.

To return to compliance and avoid further enforcement action by the EPA, you must submit a
completed service line inventory with appropriate documentation to the Division through the
Division’s lead service line inventory portal located at lcrr.utah.gov.
