Dear Mr. Giles:
On March 6, 2024, the Utah Division of Waste Management and Radiation Control (Division) received yourrequest for approval of the proposed Class IVa disposal cell at the Duchesne/WasatchBlue
Bench Landfill (DSHW-2024-005722). The Division has reviewed the design, soil sample analyses, and applicable State Administrative Rules (Rules) for consideration of the request. The
Division has determined the information provided as incomplete and requests the following information to be submitted:
CQA/CQC Plan for the facility as required by R315-310-4(2)(c)(x) as referenced in a previous request for information from the Division (DSHW-2024-208303);
Groundwater monitoring plan updates for the proposed cell location. R315-305-4(4) states that the owner or operator of a Class IVa landfill shall monitor the groundwater beneath the
landfill as specified in Rule R315-308. Specifically, R315-308-2(2) states that the ground water monitoring system must consist of at least one background or upgradient well and two
downgradient wellsbelow the facility, cell, or unit.Class IVa liner and groundwater monitoring requirements is,in part, based on the amount of waste potentially received froma very small
quantity generator of hazardous waste (see R315-305-3). Alternatively, groundwater monitoring requirements may be waived when the requirements of R315-308-1(3) can be demonstrated.
Discussion of run-on / runoff control plan per R315-303-3(c) and a map showing run-on / runoff control placement.
Apermit modification request will need to be submitted through the Community Portal (https://deqorg.utah.gov/s/login/). All associated permit modification documents should be submitted
through the Community Portal.
