Box 96 Bicknell, Utah 84715 Subject:Revised Plan Approval, Well Equipping, Brinkerhoff Well (WS009) & Well Transmission Pipeline; Bicknell Town, System #28001, File #15584, SRF #3F1786 Dear Mayor Noreen Johnson:The Division of Drinking Water (the Division) received the revised plans and specifications for equipping the Brinkerhoff Well and installing the well supply pipeline from your consultant, Brennan Rees with Ensign Engineering, on December 5, 2024. Prior plan approval for equipping the Brinkerhoff Well was issued on August 1, 2024. However, due to budget constraints after receiving bids on the project, changes were made to the well house and well supply pipeline alignment. Those revisions were submitted as part of the revised plans on December 5, 2024. Written review comments were sent to your consultant on December 11th and revised plans were received on December 19th. This project, once again, meets the conditions for receiving a plan approval. Our understanding of the changes to the project due to budget constraints is that the new well house will no longer be a masonry block building but will be a “Conex” type shipping container that will be set on a permanent foundation and modified to serve as a well house and future disinfection room. The modifications to the container provide the necessary components to meet the applicable Drinking Water Rules. The well transmission pipeline has been modified to take a more direct route to town, reducing the length of piping needed, but requires a lengthy bore under the Fremont River and adjacent wetlands. All other aspects of the project remain unchanged from the well equipping plan approval issued on August 1, 2024. We have completed our review of the plans and specifications, stamped, and signed by Brennan R. Rees, P.E., dated October 10, 2024,and found they basically comply with the applicable portions of Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the revised plans for equipping the Brinkerhoff Wellare hereby approved. This approval pertains to construction only and shall include all other requirements set forth in the plan approval letter issued on August 1, 2024. An Operating Permit must be obtained from the Director before the Brinkerhoff Well and associated pipelines may be put in service. A checklist outlining the items required for operating permit issuance is enclosed for your information.Approvals or permits from the local authority or the county may be necessary before beginning construction of this project. As the project proceeds, notice of any changes in the approved design, as well as any change affecting the quantity or quality of the delivered water, must be submitted to the Division. We may also conduct interim and final inspections of this project. Please notify us when actual construction begins so that these inspections can be scheduled. This approval must be renewed if construction has not begun or if substantial equipment has not been ordered within one year of the date of this letter.
