HomeMy WebLinkAboutDRC-2024-007901 DRC-2024- Salesforce Guidance for Uranium Recovery Facilities Applying for a New Uranium Recovery License 1) Access the Portal a) Navigate to the portal: https://deqorg.utah.gov/s/login/?ec=302&startURL=%2Fs%2F b) Login with your credentials or register if you're a new user. 2) Navigate to the Facility Section a) Click Facility from the left-hand ribbon. b) Click the Add New Facility box. c) Fill in required information. d) Click Add. 3) Navigate to the License Application Section a) Click Manage LPRs from the left-hand ribbon. b) Select the box titled “New LPR” to Apply for a New License, Permit, or Registration (LPR). c) In the Choose a Facility drop down box, select the facility created in step 2). 4) Complete Application Questions a) Program Selection: i) Choose Uranium Mills as the program. ii) Choose UMILL License as the sub-program: iii) Carefully read the Instructions. Download any necessary documents from links in the Instructions. iv) Make note of the Application Name, found in the orange ribbon neat the top of the screen. You will need this information to communicate with staff regarding your application. v) From this point on in the application process users can click Save and Exit to save their application. Users should not proceed beyond the Instructions page until they are certain that they have collected all the necessary information for their application and prepared all the necessary documents. vi) After making certain that they have collected all the necessary information for their application and prepared all the necessary documents users should click Save and Next. b) Application Questions (fields with a red asterisk are required): i) Enter Business Name registered with the Department of Commerce Division of Corporations and Commercial Code. ii) Address of Location of Use. For a Uranium Mill, this should be the site of the current or former mill iii) Name of person to be contacted for this application (A) You can either select an existing contact or create a new one. DRC-2024- iv) Radioactive Materials to be Possess – Be sure to include 11e.(2) Byproduct Material v) Purpose for which radioactive material will be possessed – For uranium mills, this should be uranium recovery vi) Name of Individual(s) responsible for radiation safety program (A) You can either select an existing contact or create a new one. vii) Click Save and Continue 5) Application Documents a) Upload any required documents specifically required in the Instructions [step 4) a) iii)] b) Upload any documents discussed during scoping meetings with Division staff c) Upload training records and other documents required for RSOs d) Upload Delegation of Authority Letter(s) for the person(s) who will have authority to request changes to the License. 6) Certify the Submission a) Check the Attestation Box to certify that the submission is accurate and complete. b) Click Save and Next. 7) Review Application Information a) Review the application on the summary page. To correct any information that is incorrect click the name of the section, such as Application Questions, from the menu on the left to return to that section and correct the information. b) Click Save and Next to move to the payment page. 8) Payment a) Submit payment as required. b) See the Community Portal User Guide for payment details & any additional questions c) When payment Status changes to Paid the Submit button will be enabled. Users MUST click the Submit button to complete the application process. Amending an Existing Uranium Recovery License 1) Access the Portal a) Navigate to the portal: https://deqorg.utah.gov/s/login/?ec=302&startURL=%2Fs%2F b) Login with your credentials or register if you're a new user. 2) Navigate to the License Modification Section a) Click Issued LPRs from the left-hand ribbon. b) Click the blue box titled Request for Amendment to the right of the License you wish to amend. 3) Completing Amendment Request DRC-2024- a) Instructions i) Please carefully read the instructions. The majority of the Amendment Request should be discussed with Division staff prior to submitting an Amendment Request b) Application Questions i) Provide a brief description of the requested changes and the rationale behind them. c) Application Documents i) At a minimum, please provide a redline-strikeout and clean copy of the requested license language under a cover letter explaining the amendment request in detail. ii) Please also submit any required supporting documentation, including, when applicable, an environmental report as required by R313-17 and R313-24. d) Attestation i) Read and acknowledge the statement. Please note that, in signing the attestation, you are acknowledging that you have reviewed all applicable rules and regulations. e) Summary and Payment i) Review the information submitted and ensure all documents are uploaded correctly ii) No payment is required for a license amendment Renewing an Existing Uranium Recovery License 1) Access the Portal a) Navigate to the portal: https://deqorg.utah.gov/s/login/?ec=302&startURL=%2Fs%2F b) Login with your credentials or register if you're a new user. 2) Navigate to the License Renewal Section a) Click Issued LPRs from the left-hand ribbon. b) Click the blue box titled Renew to the right of the License you wish to renew 3) Completing License Renewal A license renewal is, procedurally, very similar to a license application. Therefore, many of the following steps are copied from the applicable sections above. Also, similar to other licensing actions, the majority of the content of a license renewal should be discussed with Division staff prior to submitting a renewal package a) Application Questions (fields with a red asterisk are required): i) Review Business Name and review registration with the Department of Commerce Division of Corporations and Commercial Code. ii) Review Address of Location of Use is still correct. For a Uranium Mill, this should be the site of the current or former mill iii) Review Person to be contacted for this application. Please ensure the current point of contact is correct. DRC-2024- iv) Purpose for which radioactive material will be possessed – For uranium mills, this should be uranium recovery or site remediation v) Name of Individual(s) responsible for radiation safety program. Please ensure the current RSO is correct vi) Click Save and Next b) Application Documents i) At a minimum, please provide a redline-strikeout and clean copy of the requested license language under a cover letter explaining the amendment request in detail. ii) Please also submit any required supporting documentation, including, when applicable, an environmental report as required by R313-17 and R313-24. iii) Click Save and Next c) Attestation i) Read and acknowledge the statement.
