Reporting and Remediation Schedule
Dear Mr. Hvmas:
A reiease from an underground storage tank (UST) at the above-referenced site was reported on
5l13l91to the Bureau of Environmental Response and Remediation (BERR) (formeriy part of
the Bnreau of Solid and Hazardous Waste). According to Federal rule Title 40 Code of Federal
Regulations (40 CFR) Part 280 Subparts E and F, and State rule R450-202, the release must be
reported, controlled and abated, characterized and remediated, and the fuli extent and degree of
contamination must be defined. These regulations require that specific reports addressing each
aspect of the release be prepared and submitted to the Executive Secretary ruST) of the Utah
Solid and Hazardous Wastes Committee within the specific times following a release.
The Project Manager assigned to this site is PauI Zahn, and your site is identified in our recorfls
as leaking underground storage tank (LUST) site AGUP.
The attached Reporting and Remediation Schedule is issued in order to assist UST owners and
operators in evaluating and remediating releases from USTs, and enable owners and operators to
meet the requirements of applicable State and Federal reguiations.
The Remediation Schedule identifies the specific reports required for evaluating a site rvhere a
release has occurred, and the time you have for submitting those reports. The time frame is shown
in the column labeled "Compliance Day Required." The column labeled "Actual Compliance
Dates" is for your convenience in planning due dates.
Aithough Federal regulations require UST owners and operators to submit the reports rvithin
specific times following a release, the compliance dates for submitting the applicable reports may
begin when you receive this Reporting and Remediation Schedule. For example, you must submit
an Abatement and Site Check Report within 20 days of receipt of this Schedule. The "Response
Dates" are the actual dates in which you must submit the corresponding reports. This column is
also for your convenience to assist in tracking your compliance dates.
