Subject: Review Comments, Duchesne Mini Ranches 2012 Waterline Project Plan Submittal,
System #07067, File #09288
This is not plan approval.
On March 22, 2013, the Division of Drinking Water (the Division) received a partial plan
submittal for the "2012 Waterline Project" from South Duchesne Culinary Water, Inc. (SDCW).
This submittal appears to be incomplete. This letter is to provide review comments based on the
preliminary information received on March 22,2013.
We have also met with Mike Lefler, Duchesne County Fire Marshall, and JeffSchnars, on April
29, 2013 for a field inspection of the proposed fire hydrant placements for Duchesne Mini
Ranches Phase 12. Mr. Lefler was satisfied with the proposed fire hydrant placements, and
appreciated the chance for his input.
The Division needs to receive your responses addressing the following issues before the plan
review can be completed. We have reviewed the partial submittal for the SDCW "20 12 Waterline
Project" for conformance with the applicable portions of Utah's Administrative Rules for Public
Drinking Water Systems in R309, and determined that the following information is needed in
order to complete the plan review:
1. We need more details on the scope of this project (details such as which subdivision phase
this project will serve, which is apparently Phase 12 of the Duchesne Mini Ranches, but
that is not specifically spelled out in the plans or specifications or a cover letter) and how
this project fits in with the SDCW Master Plan, the total number of new service
connections that this project will add to the existing approved connections, etc. Much of
the information needed can be described in the Project Notification Form (PNF), which we
also did not receive for this project. The PNF is typically prepared by your consulting
engineer and can be found on the Division's website at:

enclosed a blank hardcopy of the project notification form for your use if you prefer.
2. Sheet 5 appears to be missing from the plans that were submitted to us.
3. Please provide details on how the proposed development relates to your existing hydraulic
model, as certified by your designated Professional Engineer (PE). We will need details
such as which lots will have acceptable flow and pressure conditions, and which lots will
not be developed due to unacceptable flow/pressure (i.e. Sheets 6 and 7 as submitted
currently show some static water pressures that are below 20psi, which is not allowed for a
point of connection). All new water system construction is required to meet the pressure
standards ofR309-105-9, Minimum Water Pressure, and that will need to be demonstrated
by your updated hydraulic model results and certified by your consulting PE. Currently
many plan sheets of your submittal have unacceptable water pressures noted on the plans,
and the context of that is not clear. An explanation of how the proposed construction
relates to your hydraulic model results is necessary for us to review for proper flows and
pressures. A written summary from your consulting engineer concerning the fire flow
scenarios that were simulated for the hydraulic model would be very helpful in evaluating
fire flows and pressure requirements.
4. Please identify as part of your submittal (in the Project Notification form or otherwise) the
PE that will be responsible for construction management and inspections of water system
construction, and for documenting the requirements for obtaining an Operating Permit
after water system construction.
