SUBJECT: STATE PROJECT NOTIFICATION FORM for Johnson Water
Improvement District (JWID) - Pleasant Valley Tank replacement, 2010

Dear Mrs. Macauley,

Included herein is the State project notification form for the Pleasant Valley Tank replacement,
2010 located in Duchesne County and prepared for Johnson Water Improvement District. The
new 100,000 gallon Concrete tank will replace a rusted out metal tank that is no longer in service,
at its existing location. The purpose of the tank is to provide water storage for the Pleasant
Valley area.

The Project Notification Form for the Johnson Water Improvement District – Pleasant Valley
Tank, 2010 has been sent to Mr. Scott Hacking, along with the Plans and specifications. I have
attached a copy of plans and specifications for your review.
