Executive Seqetary November 8,2010 Lanny Ross Johnson Water Improvement District Route 3, Box 3188 Roosevelt, UT 84066 Dear Mr. Ross: Subject: Operating Permit, 100,000-gallon Pleasant Valley Storage Reservoir (ST002), Johnson WID - Independent, System #24042, File #07855 On October 4,2010, the Division of Drinking Water (the Division) received a request for an operating permit for the Johnson V/ID - Independent (System #24042) 100,000-gallon Pleasant Valley Storage Reservoir from Korey C. Walker. P.E with Epic Engineering. The Division originally granted plan approval for this project via letter dated April 26,2010. Scott Hacking, Uintah Basin District Engineer, inspected the completed facility on October 5, 2010. Our understanding ofthe project is that the previously existing 24,000-gallon steel storage reseryoir was demolished and a new 100,000-gallon concrete storage reservoir was constructed at the same location in Pleasant Valley. The project also included the construction of a valve vault, tie-ins with existing water lines, and associated valves and appurtenances. This new water storage reservoir is referenced as ST002 in our inventory. The demolished 24,000-gallon tank (ST001) is now listed as an inactive facility. We have received the following information for the Johnson WID - Independent 100,000-ga11on Pleasant Valley Storage Reservoir: 1. Design engineer's statement of conformance with plan approval conditions.2. Design engineer's statement of conformance with the Rule for any deviation from the plan approval or plan review exemption. 3. Evidence of O&M manual delivery. 4. As-built drawings. 5. On this basis, an Operating Pcrmit for the Johnson WID - Independent 100,fi[-gallon Pleasant Valley Storage Reservoir is hereby issued as constltuted by this letter. You may now place this facility in service in your water system. Please maintain a copy of this letter with your permanent records for future reference.
