Ground Water Quality Discharge Permit UGW470001 will expire on July 31, 2025. Pursuant to UAC R317-6-6.7 and in accordance with Part IV.D of your permit, you must apply for reissuance
of the permit at least 180 days prior to the expiration date.Your permit renewal applications must address, at a minimum, the requirements of UAC R317-6-6.3 parts A through C. If any
information submitted in the previous applications have changed, that information must be updated including the types of information listed in UAC R317-6-6.3 parts D through S. An additional
but optional request is that you review the current permit language and identify any inaccuracies or items you wish to change or clarify. Woodrow Campbell will review your application
and provide you with either a proposed draft permit or a completeness review requesting additional information. After concurrence of the draft permit, a public notice will be posted
in the local newspaper to initiate the required 30-day public comment period in accordance with UAC R317-6-6.5.
