This letter is to certify that Well #2 was demolished and reconstructed according to the plans submitted
to the State of Utah’s Department of Drinking Water and meets all of the conditions of Plan Approval.
No significant changes were made during construction for the well house and pumping system of the
new well. A VFD was added and the chlorination was put in a separate room inside the well house
compared to the original well operation.

This project was bid as City project and inspected by Payson City. We have accepted the well
improvements and are ready to use the well.

Satisfactory bacteriological samples were obtained and the new lines were pressure tested prior to
putting the line into service and all aspects of the new lines meet Payson City and the State of Utah
Drinking Water requirements.

We are also providing the bacteriological samples for these waterlines and a map showing the
waterlines as-builts into our system is included with this letter.
