Dear Hunter:
In response to the plan approval letter dated January 27, 2022 and the Drinking Water Source Protection
Plan concurrence letter dated December 11, 2023 (both attached), we would like to request approval for
an operating permit for the subject project. The contractor, Corrio Construction, has completed all of the
construction for the project. The bacteria tests have been completed. The operating permit check list is
addressed below.
The following eight items address the conditions that must be met for compliance with Utah
Administrative Codes to receive an operating permit.
1.Certification of Rule Conformance by a professional engineer (P.E.) that all conditions of Plan
Approval were accomplished, and if applicable, changes made during construction were in
conformance with rules R309-500 through 550.
Bowen Collins & Associates performed limited construction observation throughout the construction
period. Bowen, Collins & Associates certifies that, insofar as can be observed, the construction of the
work conformed to the approved plans and the drinking water rules. There were minor changes made
during construction and all changes were installed in conformance with the UAC rules.
2.As-built or record drawings incorporating all changes to approved plans and specifications (unless
no changes were made to the previously approved plans during construction).
The final record drawings for the project are attached to this letter. This letter shall serve to provide
documentation that, for the Stanbury Park Improvement District Well No. 5 Equipping Project, the
approved design drawings were modified by Bowen, Collins & Associates per the project
representative’s and the construction contractor’s redlined notes for the water improvements
installation. The modified plans will serve as the record drawings for the project.
3.Confirmation that as-built or record drawings have been received by the water system.
A copy of the record drawings has been given to Stansbury Park Improvement District.

4.Satisfactory bacteriological samples as evidence of proper disinfection and flushing in accordance
with the appropriate ANSI/AWWA standards:
ANSI/AWWA C654-13 AWWA Standard for Disinfection of Wells Two consecutive samples,
none positive, no less than 30 minutes apart.
The bacteria test results are attached. Stansbury Park Improvement District sampled the water and
delivered the samples to Chemtech-Ford Laboratories. All flushing and disinfection was
accomplished in accordance with the latest applicable standards.
5.If applicable, all other documentation that may have been required during the plan review process.
No other documentation has been provided at this time.
6.If applicable, confirmation that the water system owner has received the O&M manual for the new
facility.
An O&M manual has been supplied to Stansbury Park Improvement District.
7.If applicable, location data of new storage tank, treatment facility, or source.
The well location is identified as follows:
WS005: Lat: 40°36’47.19”N; Long: 112°17’57.10”W

Dear Brett Palmer:
The Division of Drinking Water (the Division) received the plans and specifications for equipping
Well #5 and installing a new tablet chlorinator from Eric Neil, P.E., with Bowen Collins and
Associates on October 28, 2021. The Division issued Plan Approval for drilling Well #5 on
September 25, 2017. The Division received a request for an exception to R309-520-7(1)(c) on
September 28, 2021. Review comments were provided to your consultant on November 24, 2021.
The Division received revised plans and additional information on December 17, 2021. The
Division received additional information about the proposed tablet chlorinator on January 12,
2022. This letter contains the following sections:

Project Understanding
Our understanding of the project is as follows:

Equipping Well #5 (WS005)
1. This project is comprised of equipping Stansbury Park Improvement District’s new Well
#5. The new well pump, well discharge piping, and tablet chlorinator will be housed
within a new well house located near the intersection of Church Road and Old Ranch Road
in Stansbury Park, Utah.
2. The well will be equipped with a new 400-horsepower vertical turbine pump with
maximum pumping capacity of 2,500 gallons per minute (gpm). The pump will be
equipped with a variable frequency drive (VFD). The proposed intake for the pump is
approximately 250 feet below ground surface.
3. The well discharge piping will include a Rossum sand tester, an automatic backwashing
screen filter for sand removal, a magnetic flow meter, a new turbidimeter, and other valves
and appurtenances. The discharge piping will also connect to a valved pump-to-waste line
which will discharge into a detention basin outside the well house.
4. A 1.5-inch diameter water line will pull water from the well discharge piping downstream
of the sand filter and supply the new tablet chlorinator solution tank. Another 1.5-inch
diameter line will return the chlorinator water to the well discharge line just upstream of
where the piping enters the floor and flows out of the building.
5. A new Horizon Model PTF-100 tablet chlorinator will be installed inside the new well
house.
6. Well #5 will not be connected to the water system’s distribution system as part of this
project. A future water line project will connect to well discharge piping to a proposed 16-
inch diameter ductile iron pipe and into the distribution system.
7. This well is identified as WS005 in the Division’s database.
Installing Well #5 Tablet Chlorinator (TP005)
1. The proposed Well #5 Tablet Chlorinator is a tablet chlorinator using Horizon PT 90
trichloroisocyanurate (trichlor) tablets.
2. This chlorinator will add chlorine to the water from Well #5 (WS005).
3. This chlorinator is only intended to provide voluntary disinfection and to help maintain the
chlorine residual within Stansbury Park Improvement District’s distribution system. This
chlorinator is not intended to provide primary or secondary disinfection.
4. The proposed Well #5 and Well #5 Tablet Chlorinator will be connected directly to the
distribution system (DS001) in a future project.
5. The proposed Horizon PT 90 tablets meet the ANSI/NSF 60 standard.
6. The proposed design target dose of 0.3 mg/L or ppm (measured as free chlorine) is based
on an anticipated flow of 2,500 gpm from Well #5 (WS005).
7. The chlorine dose will be adjusted by an automatic flow-paced control.
8. This chlorinator is identified as TP005 in the Division’s database.

Plan Review Information
We have received the following information for Well #5 and Well #5 Tablet Chlorinator:
1. Certification of well seal.
2. Well driller’s report (well log).
3. Aquifer drawdown test results (step drawdown test and constant-rate test).
4. New Source Chemical Analyses of the well water.
5. Plans and specifications for equipping the well.
a. Pump specifications and pump curve.
b. Well head discharge piping.
c. Well house design plans and specifications.
d. Chlorinator design plans and specifications.
Plan Approval
We have completed our review of the plans and specifications, stamped and signed by Eric W.
Neil, P.E., dated October 27, 2021, and find they basically comply with the applicable portions of
Utah’s Administrative Rules for Public Drinking Water Systems in R309. On this basis, the plans
for equipping Well #5 (WS005) and installation of the Well #5 Tablet Chlorinator (TP005)
are hereby approved.
This approval pertains to construction only. An Operating Permit must be obtained from the
Director before Well #5 and the Well #5 Tablet Chlorinator may be put in service. A
checklist outlining the items required for operating permit issuance is enclosed for your
information.
Approvals or permits from the local authority or county may be necessary before beginning
construction of this project. As the project proceeds, notice of any changes in the approved design,
as well as any change affecting the quantity or quality of the delivered water, must be submitted to
the Division. We may also conduct interim and final inspections of this project. Please notify us
when actual construction begins so that these inspections can be scheduled.
This approval must be renewed if construction has not begun or if substantial equipment has not
been ordered within one year of the date of this letter.
Disinfection Requirements
Stansbury Park Improvement District is not currently required to disinfect Well #5 (WS005) based
on bacteriological source sample results currently available. The water system proposes to
voluntarily install the Well #5 Tablet Chlorinator (TP005) to help maintain a detectable chlorine
residual in the distribution system. The Well #5 Tablet Chlorinator will use Horizon PT 90
trichloroisocyanurate (trichlor) tablets. Per Division of Drinking Water policy, trichlor based
disinfectants cannot be used to provide primary or secondary disinfection. If the Well #5

(WS005) source is ever determined to require primary or secondary disinfection, the Well
#5 Tablet Chlorinator will need to be modified or replaced to comply with Division policy.
1. Chlorine Residual ‒ Distribution System (DS001)
a. This water system will be required to maintain a detectable residual throughout the
distribution system. It is recommended to maintain the chlorine residual above 0.1
ppm in the distribution system. [R309-520-5]
b. The chlorine residual measured in the distribution system must not exceed the
maximum residual disinfectant level (MRDL) of 4.0 mg/L (measured as free
chlorine). [R309-200-5(3)(c)(iv)]
c. This water system will be required to take a minimum of three (3) chlorine
residual samples per week at varying locations throughout its distribution
system (DS001). [R309-105-10(1)(c)]
d. Take distribution system chlorine residuals in conjunction with total coliform
sampling. [R309-215-10(3) and (4)]
2. Chlorine Residual ‒ The Point of Entry to Distribution System (EP005)
a. The requirements 2.b through 2.e below will go into effect when the Well #5
Tablet Chlorinator (TP005) is permitted to be connected to the system and put into
use.
b. The Point of Entry (POE) sampling location for the Well #5 Tablet Chlorinator
(TP005) will be determined when the chlorinator is connected to the distribution
system and put into use. Once determined, this POE sampling location will be
identified as EP005 in the Division’s database.
c. This water system will be required to maintain a minimum of 0.2 mg/L residual
(measured as free chlorine) at the POE sampling location. [R309-215-5(3) and
R309-215-10(2)]
d. The chlorine residual measured at the POE sampling location must not exceed the
maximum residual disinfectant level (MRDL) of 4.0 mg/L (measured as free
chlorine). [R309-200-5(3)(c)(iv)]
e. This water system will be required to record and report the free chlorine residual at
the Well #5 Tablet Chlorinator (TP005) POE sampling location (EP005) a
minimum of 3 days a week. [R309-210-8(3)(a)(ii)]
Source Protection Requirements
The Drinking Water Source Protection (DWSP) plan for Well No 5 (WS005) was due September
25, 2018. You must submit copies of Land Use Agreements that have been recorded with Tooele
County or proof that the delineation has been submitted to Tooele County to be covered under the
Tooele County Source Protection Ordinance before the well can receive an operating permit.

Temporary Exception to R309-520-7(1)(c) Chlorine Feed Capacity
R309-520-7(1)(c) Chlorine Feed Capacity states:
1. The capacity of the chlorine feed equipment shall be sized to provide at least 2 mg/L
during peak demand.
2. The feed equipment shall operate accurately over the design feeding range.
3. The feed equipment shall be designed to maintain a detectable residual at all times, at all
points within the intended area in the distribution system.
The basis for your request for an exception to R309-520-7(1)(c) is that due to the 2,500 gpm
design flow rate of Well #5 and the anticipated chemical dose of 0.2 to 0.4 mg/l, meeting the 2
mg/L requirement during peak demand would require a robust dosing pump that may not
efficiently operate at the lower range of the dosing pump capacity. You propose public health is
protected and the intent of the rule is met according to the following:
1. The dosing pump will be able to maintain a 0.2 mg/L chlorine residual at the Point-of-
Entry (POE) sampling location during peak flow.
2. The dosing pump can maintain a 0.1 mg/L chlorine residual within the distribution system
during peak flow.
3. Well #5 (WS005) is not currently required to be treated for primary or secondary
disinfection.
On this basis, an exception to R309-520-7(1)(c) Chlorine Feed Capacity, is hereby granted for
Well #5 Tablet Chlorinator (TP005).
1. This Temporary Exception expires on January 31, 2025.
2. You will be required to re-submit an exception request along with confirmation that all
conditions outlined in this temporary exception have been fulfilled and obtain an
exception renewal prior to January 31, 2025.
a. Exceptions granted by the Division include an expiration date to ensure that the
basis and conditions for granting the exception are verified periodically and that
the drinking water quality remains protected. After receiving your request, the
Division will determine whether this exception can be re-issued.
b. Failure to request the re-evaluation of this exception and obtain an exception
renewal by January 31, 2025, will result in the activation of the deficiency related
to not having an adequate means to maintain the disinfectant residual in the water
entering the distribution system.
3. This Temporary Exception may be rescinded any time if new evidence indicates that
the conditions of this exception are no longer being met or a public health risk exists
due to this exception.

Dear Brett Palmer:
The Division of Drinking Water (the Division) received the Drinking Water Source Protection
(DWSP) plan for the Well #5 from your consultant, Christopher DeKorver and Bowen Collins &
Associates on June 29, 2023, with additional information request and received on November 8, 2023.
We have completed our review of the DWSP plan, stamped and signed by Christopher J DeKorver
P.G., and dated June 14, 2023, and find that the plan basically complies with the applicable portions of
Utah’s Administrative Rules for Public Drinking Water Systems. The Division concurs with this
plan. We commend you for establishing a program to protect this source from present and future
contamination. This plan must be updated often enough to ensure that it reflects current conditions in
your protection zones.
The due date for submitting the next updated plan is December 31, 2027.
Please be aware that the source protection zones for this source have now been incorporated into the
Division’s Geographic Information System (GIS) database. In addition, these zones should now
appear on the Utah Environmental Interactive Map, so long as this system is currently classified as
Public in the Division’s database. The Utah Environmental Interactive Map can be accessed by
visiting enviro.deq.utah.gov. We encourage you to visit the map to confirm that the location of this
source and the source protection zones are accurate. If you need assistance with navigating the map or
to report any discrepancies, please call (801) 536-4200 and ask to speak to source protection staff.
Reminders
As stated in R309-600 and 605: Implementing DWSP Plans - Each Public Water System (PWS) shall
begin implementing each of its DWSP Plans in accordance with the implementation schedule within 180

days after submittal if the plan is not disapproved. Be prepared to describe these efforts in your next
update, which should include documentation of how the land management strategies identified for
existing and future potential contamination sources were implemented. Your updated plan will be
disapproved, and 25 Improvement Priority System (IPS) points will be assigned for failure to
comply with this requirement.
As stated in R309-600 and 605: Recordkeeping - As a DWSP Plan is executed, the PWS shall
document any land management strategies that are implemented. Please provide actual copies of
memoranda of understanding, public education programs, bill stuffers, newsletters, or other
correspondence documenting the implementation of each land management strategy as it occurs, in
this section of your updated plan.
Monitoring Reduction Waivers
Your request for a susceptibility waiver for volatile organic contaminants (VOCs) has been granted.
VOC sampling is now due every six years. Your request for a susceptibility waiver for pesticides has
also been granted. Two pesticide samples are now due every three years.
The monitoring schedule and associated sample due dates for all source monitoring requirements will
be specified upon the issuance of an operating permit for this source.
Use and susceptibility waivers must be renewed every six years with your updated source
protection plan, by submitting an updated waiver statement. If the update is not received, your
waivers may lapse, and your monitoring schedule will change.
