SUBJECT: No Further Action, Leaking Underground Storage Tank Release
Veterans Affairs (VA) Medical Center Building 38, located at 500 Foothill Boulevard, Salt
Lake City, Utah
Facility Identification No. 4001459, Release Site NZZ

A release of petroleum was reported to the Division of Environmental Response and Remediation
on December 17, 2020 based on samples taken at the time of tank closure. The extent and degree of
contamination has been fully defined and a No Further Action (NFA) review is hereby requested.

Case file information justifying no further action includes the following:
The VA Medical Center originally housed two 3000-gallon USTs, Tank 12 and Tank 13. On
December 17, 2020, Tank 13 was closed in place with a cement slurry, samples taken at the time of
tank closure indicated a release from the tank piping.

In May 2023, work began to completely remove both USTs and the impacted soil. Starting on May
15, the UST systems and 307 tons of contaminated soil were removed from the site as part of the
Corrective Action Plan that had been approved on March 14, 2023. Confirmation samples taken on
May 22, 2023 indicate all contamination was removed with all samples reporting non-detect for all
analytes. The excavation was backfilled with clean material and the contaminated soil was taken to
ET SRS for proper disposal.
