State St. Orem, UT 84058 Facility ID# 1000371 Release OIS Dear Ms. Brennan: The enclosed report by Maverik, Inc. (Maverik) summarizes the gasoline-fuel release that was reported to the DERR on 9/7/2023 from our Orem, Utah #314 store (795 S. State St., Facility ID 1000371) and the subsequent investigation and repairs that took place. Identification of the Release Initial alarms and subsequent line testing identified a potential release out of the premium product line on 9/6/2023. The premium line was immediately capped and taken out of service by our fuel service vendor (CSESCO), Maverik notified the DERR of the potential release, and additional line testing/verification (include inert gas testing) was conducted. The source of the leak/release was determined to be directly under the dispenser 1/2 (the dispenser furthest southeast in the canopy) as the result of a corroded flexible pipe/elbow connection (see Figure 1 below). Investigation Maverik personnel confirmed the leak with a MiniRAE 2000 portable VOC meter that showed elevated PID readings at the depth just below the elbow connection (approximately 4.5 feet below ground surface). Stained soils and a strong hydrocarbon odor were also noticed. Using hand-tools, Maverik dug out a narrow pit below Dispenser 1/2. The extent of the excavation was limited due to the presence of conduits and potential instability of the dispenser island. Four soil samples were collected for laboratory analysis from the following locations: SS-1: directly below the compromised elbow @ 4.5 feet bgs, SS-2: directly below the elbow at @5.0 feet bgs, SS-3: approximately 52 inches bgs and 32 inches northwest of the impacted elbow, and SS-4: approximately 52 inches bgs and 32 inches northwest of the elbow. The soil sample were taken to Chemtech Ford Laboratories in Sandy, UT for analysis of benzene, toluene, ethylbenzene, xylenes, naphthalene, MTBE, total petroleum hydrocarbon-gasoline range organics (TPH-GRO), and total petroleum hydrocarbon-diesel range organics (TPH-DRO). Ms. Eileen Brennan September 29, 2023 Page 3 The laboratory analytical results, as shown in Table 1, for the soil samples show concentrations above RBCA Tier 1 Screening Levels for benzene, ethylbenzene, toluene, xylenes, and TPH-GRO in soil samples SS-1, SS-2, and SS-3. All constituents analyzed for soil sample SS-4 were below detection levels. An additional soil sample was taken, identified as SS-5, from the excavated area to determine the Unified Soil Classification (USC). The results identified the material as GW, which is classified as “well graded gravel” comprised of a gravel-sand mixture with little or no fines. Release Volumes The leak was determined to have started no earlier than August 16th and had a maximum leak rate of 1.24 gallons per hour (at 50 psi in which it is tested, which is well above actual operating pressures). Inventory Reconciliation Reports for the Premium system showed that between 8/16/2023 – 9/6/2023, about 85 gallons (which was within allowable variances) were not accounted for by the dispenser sales. Waste Removal All excavated soil was collected and hauled offsite for proper disposal. In total, approximately 2980 pounds of petroleum impacted waste solids were taken to the Sump & Trap Cleaning facility in Salt Lake City. The associated waste manifest is attached. Repairs CSESCO replaced all sections of the premium line that were compromised, backfilled the area under the dispenser, and tested the line. The premium line was then put back into service on 9/14/2023. Concrete was later replaced to complete the repairs. Conclusions Based on review of the site and soil conditions, along with the analytical results, it appears that the leaked fuel went vertically, straight down. At 5 feet bgs, which was the deepest that we could excavate at the time, there was residual contamination above RBCA Tier 1 Screening Levels. A sample taken at 52 inches bgs but offset by about 32 inches to the northwest, still showed concentrations above RBCA Tier I SLs, but the concentrations were very noticeably lower than directly below the leak. A sample taken at 52 inches bgs but offset by about 32 inches to the southwest was non-detect for all Ms. Eileen Brennan September 29, 2023 Page 4 constituents analyzed. Please note that based on a previous investigation at this site (in 2009), groundwater is anticipated to be >70 feet below ground surface and therefore it is highly unlikely that groundwater was impacted.
