2123hrs
Environmental Restoration, LLC (ER) is contacted by Maverik representative, Mr. Mark Stewart, regarding
the above-mentioned project. Mr. Stewart reports Maverik store 474 has a diesel release onto the
pavement and into a drain in the lot. Mr. Stewart then requests a team to respond to the above
referenced location as soon as possible to provide labor, equipment, and materials to remediate the
release. Any generated material will be properly containerized, labeled, and taken off site for disposal by
Envirocare personnel.
2125hrs - 2230hrs
ER creates the Authorization to Proceed as well as all other electronic documentation associated with
this project. ER then informs Mr. Stewart the team will be onsite within two hours. ER spoke with the
MOD onsite, and she reported the fire department initially estimated the release to be fifteen gallons.
2345hrs
Team members arrive onsite and meet with the manager on duty (MOD). The team is briefed with the
incident details and directed to the incident area. A site-specific Health and Safety Meeting (HASP) is
conducted, and a scope of work is established.
2355hrs
The crew dons the proper PPE and begins the plan of action for this project. Mr. Stewart is then contacted
and informed the team is on-site and establishing the scope of work.
0005hrs
The RM reports that this release is much larger than originally reported. The manager on duty originally
claimed 15-gallons of diesel had released onto the pavement but the on-site personnel is estimating at
least 50-gallons of diesel has spilled onto the pavement and into the drain which is a catch basin for the
oil water separator on site. The release is an estimated ten feet wide and fifty feet long. The team will
pressure wash the pavement surface and utilize the catch basin to extract the impacted water into a vac
truck to be taken off site for disposal. The team will need to utilize a degreaser on the surface area first
and then pressure wash to remediate completely.
0030hrs
The RM states that the team already has the vac truck on site and are utilizing the vac truck to extract
liquid from the drain that was impacted that leads to the oil water separator on site. Team members are
utilizing a pressure washer to wash and remediate the diesel from the pavement, and we utilize the
backtrack to extract that impacted water to be taken off site. A degreaser is being applied as well to
break up the diesel and allow it to easily be taken off with a pressure washer. The team will begin
pressure washing at the fuel island and work their way out towards the drain.
0050hrs
The team reports they have completed this remediation process and have extracted all the impacted
liquid from the drain of the catch basin for the oil water separator. The team has filled out a BOL for the
transport of the generated material and will dispose of it as soon as possible off site. The project
generated 450-gallons of impacted water and will be disposed of offsite.
0100hrs
ER notes that all mitigation efforts have concluded, and no injuries occurred during this project. ER then
updates Mr. Stewart with the notice of project completion. The team demobilizes from site and are

enroute to refuel equipment and return to their facility. Upon arrival at the facility, the team will restock
supplies, stage equipment, and complete all project paperwork.
