This email serves to confirm the voicemail messages left for Larry Kellum and Tyler Hegburg on November 4, 2024.
Earlier today, while staging approved waste for microencapsulation, our mixed waste operations inadvertently placed two
containers from shipment 9113-35-0001 (Bates M16456) in the preparation vault before the required treatment. The waste
in shipment 9113-35-0001 includes Spent Granular Activated Carbon (GAC), GAC media, and clay balls. Treatment of this
waste was approved using Treatment Formula 9016-21 L (submitted to the Director on August 14, 2007), noting that the
treated waste may require TCLP Mercury analysis. The misplacement was identified before encapsulation began. In
response, further staging was paused, and the process to remove all staged waste, including the untreated waste, was
initiated. The removed waste is currently being held in temporary storage containers. At the end of the workday, waste
removal was paused, and the remaining staged waste in the vault was covered. Removal of the remaining waste will
resume on Tuesday morning, November 5, 2024. An investigation into the oversight is underway, and I will provide further
updates as they become available.
