Dear Vicki Savage:
On August 14,2024,the Utah Division of Drinking Water issued a Groundwater Under the Direct
Influence Determination and Order to Treat order designating your source,Mill Hollow Upper
Spring (WS001),as Groundwater Under the Direct Influence of Surface Water (GUDI),due to
conditions posing potential health risks.Using a GUDI source without proper treatment can
introduce contaminants from surface water,such as bacteria,viruses,and other pathogens,into
drinking water.This poses serious health risks,including waterborne illnesses,which must be
controlled to protect public health.
Please be advised that the original GUDI Order,dated August 14,2024,remains final and in
effect.This letter does not close the Order or its requirements.All obligations outlined in the
original Order,including those related to source inactivation and compliance with surface water
treatment standards,must still be met fully and promptly before the source can be returned to
service.Compliance with the temporary inactivation option in no way releases the system from
the ongoing requirements and timelines set forth in the original Order.
We received confirmation on August 22,2024,of the Mill Hollow Upper Spring (WS001)
temporary inactivation and physical disconnection,as outlined in option 31(c)of the Order.Under
this option,you must maintain the source's physical disconnect from the distribution system while
working on a long-term solution.Please note the following requirements to ensure compliance
with the Order:
1.Notification Requirement:Provide the Division with at least sixty (60)days'notice
before reactivating the source.
2.Approval for Reactivation:Written approval from the Director is required prior to
reactivating the source.

Additionally,before reactivating the source for drinking water purposes,your system must fully
comply with all surface water treatment requirements detailed in 31(a)of the Order.This includes
installing and operating a Division-approved surface water treatment facility that meets necessary
standards.Reactivating the source without meeting these requirements could lead to immediate
enforcement actions,including a "Not Approved"rating or other corrective measures deemed
necessary by the Division to protect public health.

long-term solution for the safety of your water system.
