R307-415-6a (3)(cXii), the following deviation report is submitted.

Permit conditions for which deviation occurred:
l!.B.3.a. Operating temperature shall be no less than 1,360 degrees F.

Describe the deviation that occurred:
RTO temperature fell below 1,360 degrees F for approximately 30.5 hours.
Production activities were halted, and lines connected to the RTO remained
down until repairs were completed.
Wasthe deviation dueto and "upsetcondition" asdefined in Section ll of the permit.

Replaced with new rod sensor and flame sensor.
6 Describe any corrective actions or prevention measures taken:
Restarted without incident.
