Hey Chris,
The attached spill report was forwarded to my attention today.
These types of releases are covered under condition I.6 of DAQE-AN112340015-24 (copy attached) of the Approval
Order for this source location. I have also attached a copy of the rule (R307-107) incorporated by reference as part of
condition I.6.
In the future, you can send these types of notifications directly to me including the information referenced in R307-107-
2(1) (a) through (f) and that will satisfy the requirements of Utah Air Quality rules and the applicable Approval Order.

Chad,
Thank you for the clarification. If it doesn’t say in the DERR report, I reported the release within 24 hours of
my notification of the event, even though it was past the timeline of the event itself.
We’re still investigating the cause of the release, so the written report will reflect that.

Re: Point of the Mountain Release

Dear Chad,

On Tuesday November 5th, 2024 between 10:30 and 11:00 AM, Staker Parson Companies’
ready-mix concrete plant located at 11700 N Frontage Rd, Lehi, UT experienced a pressure
failure in one of the vertical cement powder storage silos on the roof of the plant.

The cause of the failure is currently unknown, but a small hole roughly an inch in diameter
caused around 5 tons of cement powder to blow out of the silo onto the surrounding yard.
The duration of the release was between 15 and 20 minutes, and the wind was blowing
East-bound at the time. When the release was discovered, operations were halted at the
plant to accommodate cleanup. The vast majority of the powder fell to the ground onsite.

The Staker Parson environmental team was not informed of the release, so no report was
filed with UDEQ within 24 hours as required by Utah administrative rule R307-107 and part
I.6 of DAQE-AN112340015-24. The team was eventually notified after business hours on
11/15. A verbal report was made with the UDERR afterhours spill hotline on 11/18.

While the investigation into the cause of the release is still underway, the only affected silo
was a newer aftermarket Erie unit, unlike the originals still on the plant roof. This may or may
not have played into the cause of the release.

Since the release, the plant has repaired the cement powder silo, but operations remain
paused while unrelated maintenance is performed onsite. While the release was an
unforeseeable failure, Staker Parson Companies’ environmental team has communicated
with site personnel the urgency required when internally reporting any environmental
incident.

At the ready-mix concrete plant, one of the 30,000 ton cement powder storage silos at the top of the plant began leaking at around 10:30 am on
11/5/2024. The release occurred for approximately 20 minutes. The cause of the leak is still unknown. Approximately 5 tons of cement powder were
released at the site. The wind was blowing east at the time and the responsible party is stating that most of the release was contained to the Staker Parson
property boundaries. No storm drains were impacted.
