A process was established by LiquaDry that we would perform total coliform testing weekly, but a
sample was missed for the weeks of January 15th, February 12th, and May 6th. Since samples were
consequently taken in the following weeks, at this time additional tests will not compensate for the
missed tests and no extra testing is scheduled. All other tests have been completed as required and no
coliforms have been detected since the water system was established.
Because we take samples each week, for the month of May testing included the required four samples
even though a week was missed.
The week of January 15th was Martin Luther King holiday and key personal had taken PTO. This created
some confusion, and the sample for the week was not sent for testing. A similar confusion occurred the
week of February 12th except the holiday was Valantines. Shortly before May 6th, a key person involved
with ensuring the test was completed had resigned from the company leading to another missed
sample.

At the time samples were not sent for testing, it was unknown by key personnel that when a week was
missed an additional sample could be tested the following week. Had the requirement been better
understood by key personnel when the scheduled weekly test was missed, a sample would have been
sent the following week to compensate and be compliant. Future occurrences of this non-compliance
are unlikely.
Multiple improvements have been initiated to reduce the possibility of future non-compliance including:
additional training; additional personnel involvement; and electronic reminders.
As of June 1st, our requirements have changed. Total coliform is to be tested once each month instead
of four. This may or may not change how often we choose to test, but coliform testing will be done at
least once each month.
