I found this boil order that we have used in the past. Let me know if this will work. Like I said in my last
email, we want to wait to post this unl we have our sample results tomorrow. Can I wait to submit the
cerficaon for boil order unl we get our test results?

My email address was not correct in the other thread. Here is a picture of the locked hydrant. I will get back to you in
the morning about the boil order. We feel we should hold off on posting the physical boil order until we have the results

Hi Bryce,
Just following up to our phone conversation. This is the email chain you can respond with the picture
of the locked hydrants. Per our conversation, we are moving forward with these locked 4 hydrants
(only source of water on property) and a boil order posted. Once we receive the results on Friday,
depending on those results you will move forward with broadcast if needed (positive repeat results).
Attached are the three documents needed for completinga boil order:
Boil Order Example: This is going to help you to write your own boil order. Feel free to work
on this document or the Boil Order PN if you would like.
Boil Order PN: This is the template we have for systems to fill out their information and send
it back for approval.
Division of Drinking Water Boil Order Instructions: This will provide you with instructions on
many things boil order related (including ways to deliver notice). The 3rd page of this
document is the certification form that needs to be filled out after you deliver the boil order to
the customers.
Certification for Boil Order: This is the above 3rd page just pulled out of that document and
created as its own document to submit
The Boil Order needs to be filled out and sent back to me for approval. Once it is approved you can
move forward with delivering it to the customers. The Boil Order needs to be delivered to the
customers within 24 hours.
Please send us the boil order template back for approval as soon as possible. We just want to make
sure it gives you all time to deliver it to the customers before the 24 hour deadline tomorrow. I will
keep an eye on it throughout today. If I do not see it by tonight, I will check back again tomorrow
morning.
