We dropped off 2 samples yesterday, Expecting the same results.
Replaced the entire faucet assembly on the employee sink today, Checked all visible and assumed Water
lines leading to employee sink, No breaks or CC points found. .
Today he's taking up, 4 samples from across the property,
JJs(US), Employee housing(US) , employee sink and F building (DS)

Thanks for getting the template back to me. Attached you will find the following,
Approved UTAH10033 Boil Order public notice for UTAH10033 along with some instructions.
Division of Drinking Water Boil Order Instructions: This will provide you with instructions on
many things boil order related (including ways to deliver notice). The 3rd page of this document
is the certification form that needs to be filled out after you deliver the boil order to the
customers.
Certification for Boil Order: This is the above 3rd page just pulled out of that document and
created as its own document to submit
