This gap analysis includes a review all local, state, license, provincial and/or procedural requirements. 1.1.2 This work instruction is to document the process for developing and maintaining a list of ladders. 1.1.3 This work instruction is to document the process for performing ladder inspections. 1.1.4 The purpose of this work instruction is to provide detail on the site process used to train personnel on ladder safety. 1.2 Scope 1.2.1 This covers the requirements in ES-SH-PR-113 for site designated work instructions CL-SH-WI-113-01, CL-SH-WI-113-02, CL-SH-WI-113-03, and CL-SH-WI-113-04 2 REFERENCES 2.1 Reference 1 [ES-SH-PR-113, Ladder and Stairway Safety] 2.2 Reference 2 [CL-TN-PR-100, Clive Facility Training] 3 REQUIREMENTS AND GUIDANCE 3.1 Applicable Sections/ Additional Requirements (01) 3.1.1 After thorough procedure and regulatory requirement review all sections of ES-SH-PR-113, Ladder and Stairway Safety are applicable and are addressed in this site-specific work instruction. 3.1.2 There are no additional local or state regulatory requirements that apply to this procedure. 3.2 Process for Maintaining a Ladder Inventory (02) 3.2.1 All Portable Ladders will be tracked and maintained in an inventory that must, at a minimum, include: Facility Specific Work Instruction CL-SH-WI-113 Rev 1 Ladder and Stairway Safety Sections/Additional Requirements Non-Proprietary Page 3 of 9 3.2.1.1 Ladder ID, Size/Length, Type, Construction Material, Rating, Storage Location, Date Purchased, Date Removed 3.2.2 All Fixed Ladders will be tracked and maintained in a list that must, at a minimum, include: 3.2.2.1 Ladder ID, Size/Length, Location, Construction Material, Date Purchased, Date Removed 3.2.3 Ladders may be tracked using CL-SH-WI-113-F1, Ladder Inventory or equivalent form by the Health and Safety department. 3.2.4 Ladders purchased before April 2024 may be marked as “unknown” for the purchase date in CL-SH-WI-113-F1, Ladder Inventory. 3.2.5 All new ladders and stairways purchased and brought on site must be added to the site’s inventory list and shall be reported to the Health and Safety department. 3.3 Process for Maintaining, Inspection, and Storing Ladders (03) 3.3.1 A visual pre-use inspections shall be conducted on all ladders before each use by the employee utilizing the ladder and shall take into consideration: 3.3.1.1 Loose, cracked, split, or broken steps, rungs, side rails or braces. 3.3.1.2 Loose rivets, screws, or bolts 3.3.1.3 Slivers on side rails, rungs, or steps 3.3.1.4 Loose, missing, worn, or damaged ladder safety shoes 3.3.1.5 Missing or loose extension locks 3.3.1.6 Frayed or broken ropes 3.3.1.7 Loose, bent, or broken hinge spreaders or loose hinges 3.3.1.8 Puncture and laceration hazards 3.3.1.9 Manufacturer safety labels are attached and legible. 3.3.2 An annual ladder inspection shall be conducted by each department for the ladders in their area using CL-SH-WI-113-F2 and turned into the Health and Safety department for tracking on the inventory. 3.3.2.1 The Safety Department will send out the current inventory Facility Specific Work Instruction CL-SH-WI-113 Rev 1 Ladder and Stairway Safety Sections/Additional Requirements Non-Proprietary Page 4 of 9 to initiate ladder inspections for the year. 3.3.2.2 Ladders that pass an annual inspection shall receive a zip tie or other color marking indicating the inspection was completed for the year. 3.3.2.3 The site shall use the same color utilized for other inspections including fall harnesses and below the hook lifting devices in the same calendar year. 3.3.3 Ladders that fail an inspection shall be tagged out immediately by the employee conducting the inspection and the employee shall report it to their direct supervisor. 3.3.3.1 Supervisors may replace the stickers as needed on ladders in their areas. 3.3.3.2 Supervisors shall put in the appropriate maintenance request for tagged out ladders if allowed in the manufacturer’s guidelines. 3.3.3.3 Supervisors shall dispose of tagged out ladders that are not able to be repaired and notify the site Safety and Health department to remove the ladder from the inventory. 3.3.4 The Health and Safety department will update the inventory as needed based on the inspection sheets from each department. 3.4 Process for Conducting and Documenting Training (04) 3.4.1 All employees shall receive ladder safety training during new hire and annually through a toolbox talk in a weekly safety topic. 3.4.2 All employees shall receive general awareness training on ladders and stairways during new hire and site annual refresher training. 3.4.3 Training shall be documented according to CL-TN-PR-100, Clive Facility Training 3.5 Records 3.5.1 Completed inspection forms shall be submitted to the Health and Safety department by the responsible supervisor/manager over each department.
