The Logan Landfill will be closed in a series of phases starting the summer of 2020. The primary goals of the quality assurance program are to: • Determine if proper construction techniques, materials, and procedures are used. • Determine if the intent of the construction documents is met. • Identify construction problems and provide a method for resolution of any problem. • Document the construction activities. Upon the completion of each final cover phase construction; a final construction documentation package will be prepared and submitted to the State of Utah Division of Waste Management and Radiation Control (UDWMRC). 1.2 Document Format The Quality Assurance Plan is divided into three sections to identify; 1) the general information and personnel associated with the project; 2) the detailed requirements of the quality assurance program; and 3) the documentation and record keeping associated with the construction. 1.3 Definitions The following are definitions that are utilized in this document and without definitions may be taken out of context: As-Built Drawings – Drawings recording the constructed dimensions, and details. ASTM – American Society for Testing and Materials. Contractor – The person or firm (to be determined) responsible for the placement of cover soils. Engineer – The person or firm (IGES Inc.) responsible for the development of the Quality Assurance Plan. Landfill Manager – Owner’s representative on site (Tyler Richards), who is responsible for all construction activities. Nonconformance – A deficiency in characteristic, documentation, or procedure that renders the quality of an item or activity unacceptable or indeterminate. Examples of 2 nonconformances include, but are not limited to, physical defects, test failures, or inadequate documentation. Owner – Logan City. Project Documents – Drawings and this quality assurance plan. Quality Assurance/Quality Control – A planned, and systematic pattern of procedures and documentation designed to provide adequate confidence that materials or services meet contractual and/or regulatory requirements, and that these materials will perform satisfactorily in service. Quality Assurance/Quality Control will be performed by the Owner. Quality Assurance Inspector – The Quality Assurance Inspector for the final cover placement will be the Landfill Manager. Quality Assurance Manager – Not utilized for this project. Quality Control Inspector – The Quality Control Inspector will be a representative of the Contractor responsible for the day-to-day Quality Control activities performed on- site. Quality Control Manager – The Quality Control Manager will be responsible for implementing the overall Quality Assurance Plan and be responsible to oversee the Quality Control Inspector. Quality Assurance Plan - A planned series of observations and tests designed to provide adequate confidence that materials and services meet contractual and/or regulatory requirements. State of Utah – State of Utah Division of Waste Management and Radiation Control (UDWMRC) is the agency responsible for permitting landfills in Utah. 1.4 Meetings Preconstruction and postconstruction meetings will be held in association with each Phase of the final cover construction at the Logan Landfill. The meetings are intended to facilitate the cover construction and minimize the potential for errors. The Landfill Manager will conduct and document all meetings. A. Preconstruction Meeting – A preconstruction meeting will be held with representatives from the Owner, UDWMRC staff, the Engineer, and the Contractor (if selected prior to the preconstruction meeting). The “for construction” documents, quality assurance plan, testing requirements, reporting procedures, and overall project responsibilities for all parties will be reviewed at the meeting. B. Progress Meetings – Progress meetings are not anticipated. Progress meetings will be scheduled as necessary. If a progress meeting is scheduled, representatives from the Owner, State, the Engineer, and Contractor will attend. The purpose of the progress 3 meetings will be to discuss progress, identify problems (and corrective actions), review test data, and coordinate work activities including testing. C. Postconstruction Meeting – A postconstruction meeting will be held with representatives from the Owner, State, the Engineer, and the Contractor (if available). The final cover construction work and documentation will be reviewed and compared to the Quality Assurance Plan for completeness. If adequate documentation is not included, test pits may need to be excavated to demonstrate cover soil thickness. 1.5 Responsibilities of Personnel A. Responsibilities of the Landfill Manager – The Landfill Manager is the primary Owner’s representative on the project. The Landfill Manager defines the overall project scope and is responsible for the implementation of all construction related activities. The Landfill Manager is the point of contact for all communications. The Landfill Manager reports to the Environmental Director at the City of Logan. B. Responsibilities of the Quality Assurance Inspector – The Landfill Manager will function as the Quality Assurance Inspector and be responsible for all on-site construction activities. C. Responsibilities of the Quality Assurance Manager – Not utilized for this project. D. Responsibilities of the Quality Control Inspector – The Quality Control Inspector will be a representative of the Contractor responsible for the day-to-day Quality Control activities performed on-site. E. Responsibilities of the Quality Control Manager – The Quality Control Manager will be a representative of the Contractor responsible for implementing the overall Quality Assurance Plan and be responsible to oversee the Quality Control Inspector. The Quality Control Manager will be responsible to document all Quality Control activities once construction is complete. F. Responsibilities of Engineer – The engineer is the person or firm (IGES) responsible for the development of the quality assurance plan and will be available for clarification of the plan during construction if necessary. G. Responsibilities of UDWMRC – The UDWMRC is the permitting agency responsible for the oversight of all landfilling activity in the State. UDWMRC is responsible to review and approve the quality assurance plan developed by the engineer. The UDWMRC will be available for clarification of the approved plan or State related concerns if necessary. 1.6 Construction Documents A. QA/QC Plan – Appendix A contains the Class I Landfill Final Cover QA/QC Plan for the first Phase of the final cover construction at the Logan Landfill. The QA/QC Plan includes the construction drawings as well as revegetation specifications. A preconstruction survey of the existing landfill cover (in the area to be closed) may be utilized to document the preconstruction grade and serve as a basis for a post construction 4 survey utilized to demonstrate cover thickness. Alternatively, Contractor may elect to install grading stakes to indicate required cover thickness. B. Specifications – The only project specifications for the cover construction are presented in Part 2 and on the drawings in Appendix A. C. Forms – The forms required for use on the first Phase of the final cover project are included in Appendix A. 5 PART 2 – CONSTRUCTION QUALITY ASSURANCE 2.1 Introduction This section describes the monitoring and testing that will be performed to assure that the final cover construction meets the specified requirements. 2.2 Material Evaluation A. Testing – Soil sources will be identified prior to placement. No specific testing of the soils to be utilized in the construction will be required. The soil to be utilized for the first Phase of the final cover at the Logan Landfill will come from the soils excavated during the construction of the new wastewater plant. B. Test Fill – No test fill will be required for this project. 2.3 Construction Testing A. Test Procedures – Soil will be tested according to the tests identified in Table 1. B. Test Frequencies – Soil will be tested according to the minimum frequency identified in Table 1. 2.4 Monitoring Requirements The monitoring requirements will include verification of final cover thickness. 2.5 Documentation and Control of Nonconformance 2.5.1 Observation of Nonconformance Whenever a nonconformance (cover thickness less than 24”) is discovered the Landfill Manager will either document the location of the nonconformance (to be corrected at a future date) or correct the cover thickness nonconformance as they are discovered. 2.5.2 Determining Extent of Nonconformance Whenever a nonconformance is discovered or observed in the construction process, product, documentation, or elsewhere, the Landfill Manager will determine the extent of the nonconformance. The extent of the deficiency may be determined by additional sampling, testing, observations, review of records, or any other means deemed appropriate. 6 2.5.3 Documenting Nonconformance All nonconformances (observed but not corrected) will be documented in writing on a Problem Identification / Corrective Action form. The documentation will occur immediately upon determining the extent of the nonconformance. 2.5.4 Corrective Action Since the only criterial item required for final cover construction documentation is cover thickness; the prescribed corrective action is to add additional soil until a minimum of 2’ of soil cover is documented. For those nonconformances not relating to cover thickness (drainage features), the engineer will be asked to review the proposed corrective action. The UDWMRC will be notified of all corrective actions (other than soil thickness) prior to implementation. 2.5.5 Verification of Corrective Actions The Engineer and UDWMRC personnel will verify all corrective actions. The verification will be accomplished by site observations, depth testing (test pits), and photographs. Verification of corrective actions will be made during the postconstruction meeting. Written documentation of the corrective measures will be made by the Engineer on the second page of the Problem Identification / Corrective Action form. 2.6 Construction Surveys A pre and post-construction survey may be performed by the Owner to document final cover geometry and thickness. If grading stakes are utilized, construction surveys are not required. 2.7 Revegetation Contractor to perform revegetation as detailed on the drawings in Appendix A. 7 PART 3 – DOCUMENTATION 3.1 Recordkeeping The quality assurance plan relies on the observations, testing, and associated documentation of all construction activities. The documentation activities will be performed by the Landfill Manager. A. Daily Record of Construction Progress – Due to the anticipated method and rate of construction; daily construction reports may not be utilized. Soil will be moved by the Contractor and potentially stockpiled on the existing landfill for several days before being graded down the landfill slopes. Once enough material has been stockpiled; the Contractor will grade the stockpiled material out to form the 2-foot thick final cover. B. Weekly Record of Construction Progress – If the final cover construction is done at a relatively slow pace; the Landfill Manager will fill out weekly construction records (during the weeks that the final soil cover materials are placed) detailing the number of loads of soil hauled and a summary of test performed. Problem Identification / Corrective Action – The problem identification / corrective action form is to be utilized to identify and document portions of the construction that do not meet project specifications. The problem identification / corrective action form also documents the corrective action associated with every problem identified during construction. The Landfill Manager and Engineer will be responsible for filling out the Problem Identification / Corrective Action form. D. Photographs Construction activities will be photographed. Photographs will include any significant problems encountered, show areas of potential corrective actions, and document construction progress. The photographs will be identified by location, date, and a brief narrative of the subject of the photograph. The Landfill Manager will be responsible for taking and cataloging construction photographs. 3.2 Final Construction Report At the completion of each Phase of the final cover construction, the Landfill Manager will submit a final construction report to the Engineer. This report will document that the final cover has been constructed in compliance with the construction drawings and specifications. 8 At a minimum, the final construction report will contain: • A summary of major construction activities • A summary of field tests (if any) • Sampling and testing location drawings (if any) • As-built record drawings (if prepared) • Photographs of the final cover construction Once the Engineer reviews the construction report from the Landfill Manger and deems that the documentation is complete and that the work product has met the intent of the design, he will issue a letter to the UDWMRC. The letter to the UDWMRC will contain a statement of compliance with the construction drawings and specifications signed and stamped by a professional engineer registered in Utah. The post construction as-built survey drawing (if utilized) will present the location of all tests performed associated with the cover construction. The soil samples tested all classify as either CL (inorganic clays of low to medium plasticity) or CH (inorganic clays of high plasticity or fat clays) with Plasticity Index ranging between 26 and 30. The bulk of the soils were finer that a #200 sieve with no less than 89.1 percent passing the #200 sieve. The soils tested for use in the Phase 3 cover will meet the requirements of R315-303-3(4). The laboratory testing results area are attached. Since the construction of the Phase 3 final cover is exactly the same as the Phase 2 and Phase 1 final covers (with the slight difference in the cover soils), Logan City would like to utilize the previously submitted Phase 1 QA/QC plan and applicable portions (cover detail and specifications) of the Phase 1 Closure Plan. The contractor that installed the Phase 1 and Phase 2 covers is scheduled to construct the Phase 3 cover. Additionally, a new aerial topography survey for the Logan Landfill has been completed to aid in the Phase 3 cover construction. The updated topography as well as the final cover contours and conceptual closure sequence is attached for your review and approval. The soil samples tested all classify as either MH (inorganic elastic silt of high plasticity) or CH (inorganic clays of high plasticity or fat clays) with Plasticity Index ranging between 17 and 47. The bulk of the soils were finer that a #200 sieve with no less than 68.1 percent passing the #200 sieve on any of the soil tested. The soils tested for use in the final landfill cover will meet the requirements of R315-303-3(4). The laboratory testing results area are attached. As the memo and test data suggest, these soils are similar to those soils previously approved for the final cover at the Logan Landfill. The request to use these soils for final cover at the Logan City Landfill is hereby approved.
