Semi-Annual Title V Deviation Report
In accordance with Operating Permit provision I.S.2.b, and Utah Administrative Code (UAC)
R307415-6a(3)(c)(ii), the following deviation report is submitted:
l. List the dates that the deviation occurred during reporting period:
Start: July 31,2023
End: December 31, 2023.
Permit condition for which deviations occurred:
Condition II.B.4.e.3, condition II.B.6.b.3, condition II.B.6.d.3., condition II.B.6.9,
II.B.6.k.3., condition II.B.6.1.3., condition II.B.6.m.1, condifion II.B.6.m.3., and condition
I.S.2.c.
Describe the deviation that occurred:
Holcim did not submit results of stack testing and RATA testing within 60 days, failure to
submit a complete and accurate SEDR for 2Q2023 and 3Q2023 on or before the required
due dates for SO,, NO,, Hg, THC, and HCI monitoring systems (already being processing
underNOV), late HCL spike testing in accordance with the requirements of appendix F to
40 CFR part 60 for HCI relative acctracy testing, exceedence of the applicable D/F
temperature limits for an intermittent period on July 18, July 20, and, July 21,2023.
Was the deviation due to an "upset condition" as defined in Section II of the permit?

Describe the probable cause of deviation:
Managernent of Change. New flurnagement and new plant environmental manager who did
not effectively oversee execution of above reports. Temperature limits were quickly
corrected and alarms verified to be functioning.
Describe any corrective actions or preventive measures taken:
Developed compliance and reporting calendar matching permit and reporting requirements,
added layers of administrative oversight and electronic tracking database.
