Dear Bruce Bower:
The Division of Drinking Water (the Division) received your request for an Operating Permit for the
Paradise West Subdivision Water System on June 30, 2021. This water system has been in operation
since about 1997 as a non-public water system.
On May 8, 2020, a “Public Drinking Water Requirements” letter was sent to you, outlining
requirements for becoming a Public Water System. The project was originally assigned to Mike
Newberry and then transferred to Hunter Payne. On March 1, 2021, Hunter Payne conducted an
informal site visit to look at the drinking water facilities and provide recommendations to meet the
drinking water requirements. On March 2, 2021, the project was transferred to Dani Zebelean.
On August 10, 2021, a sanitary survey was conducted by Dani Zebelean and Hunter Payne. Some
deficiencies and recommendations were issued because of that survey. Many items from the previous
year’s site visit had been fixed, but four items still had not been addressed. Three items are significant
deficiencies. The system currently does not use an NSF/ANSI 60 certified chlorine tablet to disinfect.
The system batch disinfects the storage tank. The system will look at receiving a CA/EO in order to
replace the chlorinator with an approved facility. Finally, there are five existing home booster pumps
as these homes are built above the elevation of the storage tank. The system will pursue going through
the exception process for the booster pumps.
Because this system had been in operation since 1997 and the facilities and subdivision were
constructed between 1997 and 2020, some of the facilities and plans had been reviewed and had
received only plan approval (approval for construction).
File #04945 — 0.12 MG Tank and Pipe Line Project. Plan approval was issued on May 6,
1998. Only the storage tank was constructed out of this project. A site visit was conducted on

August 10, 2021 which found the tank met the current R309 Rules. An Operating Permit
will be issued with this letter.
File #12005 – Source Review (WS001). A current Drinking Water Source Protection Plan and
recorded Land Use Agreements have been received by the Division. A site visit conducted on
August 10, 2021 found the well to be constructed to meet the current R309 Rules. An
Operating Permit will be issued with this letter.
File #12006 – Engineering review of the Paradise West Subdivision. As-built drawings for the
distribution system were provided by Chris Wright, P.E. of Hansen & Associates, Inc. An
onsite inspection was performed on August 10, 2021. Based on the site visits, submittal of as-
built drawings, and corrections that have been made, an Operating Permit will be issued
with this letter.
We have received the following information for the Paradise West Subdivision:
1. Record drawings.
We have determined that all conditions for operating permit issuance have been met. On this basis, an
Operating Permit for the Paradise West Subdivision is hereby issued as constituted by this
letter.
The issuance of this Operating Permit resolves your water system’s deficiencies of G001 and S001
related to unapproved facilities and a source in service. An updated IPS report is available at
https://waterlink.utah.gov. Please contact Jennifer Yee at (385) 515-1501 or jyee@utah.gov for
questions related to your water system’s IPS report or deficiencies.
Please maintain a copy of this letter with your permanent records for future reference.
