OnNovember 8, 2024,Shaunna Millsfrom the Utah Division of Water Quality (DWQ) performed a compliance evaluation inspection on the subject site, pursuant to the Utah Common Plan Permit
(CPP) UTRH00000. The purpose of this inspection was to ensure compliance with the permit conditions and to evaluate the effectiveness of the implemented Storm Water Pollution Prevention
Plan (SWPPP).The following violations were identified during the inspection:
The site operator has not obtained permit coverage under the Utah Common Plan Permit (CPP)(Utah Administrative Code R317-8-3.9(1)(a)2., (6)(c), (d)10., and (6)(e)l.).
Action Required: If the operator does not believe they meet the eligibility conditions, please provide documentation as to the reason why permit coverage is not required. Otherwise,
obtain coverage under the CPP by submitting a Notice of Intent (NOI). The NOI can be obtained online at the NeT Online Permits Database, which is found at: https://deq.utah.gov/water-quality/general-construction-storm-water-updes-permits
A notice of permit coverage was not posted in a conspicuous, safe, publicly accessible place near the entrance to the project (CPP Part 1.9).
Action Required: Post a sign in a conspicuous, safe, publicly accessible place near the entrance to the project. Ensure the font on the sign is large enough for normal corrected vision
to easily read the sign contents from a public right-of-way.
No Storm Water Pollution Prevention Plan (SWPPP) was created (CPP Section 4).
Action Required: Develop a SWPPP consistent with the requirements in Section 7 prior to the submittal of the Notice of Intent NOI. Ensure the SWPPP is kept up to date throughout the
duration of permit coverage. A SWPPP template can be found at:

No perimeter controls installed, to prevent sediment from leaving the site(CCP Section 2.1). Action Required: Install and maintain appropriate perimeter controls.
No track-out pad installed, to prevent dirt/mud from being tracked out onto the road (CCP Section 2.4).Action Required: Track-out pads must be used to prevent dirt/mud tracked on streets
as vehicles leave the site. If traffic onto and off the site is not frequent, a site operator may impose a blanket prohibition of vehicle traffic onto the site, allowing for the occasions
to deliver and unload, but afterwards providing sweeping and/or cleaning of tracked out dirt. Track-out pads (see definitions) and or rumble strips (see definitions) must be used to
prevent dirt/mud tracked on streets as vehicles leave the site. If traffic onto and off the site is not frequent, a site operator may impose a blanket prohibition of vehicle traffic

deficiencies 1-3 and photographicevidence of the corrected items for deficiencies 4 & 5 to the DWQ inspector via email within 7 days of the receipt of this letter. All violations are
subject to enforcement action; failure to complete the corrections within the timeline may result in a referral to our compliance and enforcement section.

Perimeter controls are not being used on the South or East side of the project; perimeter controls must be installed in both areas. The North side of the project has been fully stabilized
