AECOM mobilized to the UTTR-North in April 2024 to document the condition of each site with regard to the institutional controls required by the SMP. The site inspections included locating and inspecting the corner markers; evaluating site conditions for change; and inspecting/repairing site management signs. A summary of inspection results is included in Table 2-1. During the inspection, 42 new soil piles were observed on the edge of N002, within the boundary of the corner markers. Following a base investigation, it was determined that the piles were scraped from the test pad of a nearby weapons test site in the Cluster Bomb Unit (CBU) Valley. It is recommended that the soil piles be left in place at N002 since there are potential risks associated with moving the piles and leaving them in place would not change the current risk evaluation in the Revised Risk Assessment Report (RAR) (AFCEC, 2023). It is also recommended that the faded site number stickers at N030M and N030S be replaced during the next inspection. It should also be noted that in 2016 groundwater monitoring wells were installed at N014, N015, and N024 in support of the RAR (AFCEC, 2023). Groundwater sampling is not currently required at these sites; however, it is recommended that the annual inspections include verification of surface completion and well cap integrity to maintain the surface seal of the three wells. No other recommendations were made for the SWMUs based on this inspection. The SMP requires specific institutional controls designed to reduce risk to humans and/or ecological exposure. The institutional controls implemented at the 25 SWMUs representing 35 locations are detailed in the SMP and include the following: • Fencing to restrict access to site by ecological and human receptors. • Signage to inform personnel about land use restrictions. • Corner markers to define “No Dig” areas where buried waste is known or suspected. • Land use restrictions to prevent unauthorized construction, grading, excavation, or other soil disturbance activities. • Site inspections to verify that site conditions remained unchanged. • Groundwater sampling at two locations (N004 and N060) • Annual reporting of any findings following site inspections. A total of 44 SWMUs are addressed under the jurisdiction of the Resource Conservation and Recovery Act (RCRA) in Part B, Module IV of the Thermal Treatment Unit (TTU) operating permit (United States Air Force [USAF], 2013). Of the 44 RCRA SWMUs, 19 SWMUs were accepted for no further action or have been transferred to another module of the permit; the remaining 25 SWMUs were recommended for site management (AFCEC, 2014b). This Management Activity Report describes the 2024 inspection for the 25 SWMUs representing 35 locations (subdivided areas within SWMUs used to better define features within a site). Groundwater sampling is no longer conducted as discussed in Chapter 3. It should be noted that six of the SWMUs (locations N004, N014, N015, N020, N021, and N024) were previously in a corrective action program with the intent to achieve site closure in accordance with USAF and Utah Department of Environmental Quality (UDEQ) requirements. These six SWMUs were included in the periodic inspections and maintenance of institutional controls already in place (i.e. signs). However, it was determined in 2019 that these sites are not likely to achieve closure, and, therefore, the institutional controls recommended by the SMP will be applicable for subsequent years. Annual site inspections are also used to implement repairs/replacements necessary for institutional controls as described in the following sections. 2.1 INTERIM MAINTENANCE No interim maintenance was conducted between the 2023 and 2024 site inspections. 2.2 2024 INSPECTION ACTIVITIES AECOM submitted a Visit Request Form to the Hill AFB Project Manager listing all of the required information for each member of the field team two weeks prior to field activities. Site inspections were conducted on April 15, 16, and 17, 2024, by Amanda Grapes Dellinger of AECOM and Elizabeth Tevault of AFCEC/CZO. Personnel were accompanied, where required, on April 16 and 17, 2024, by Ward Stern, an unexploded ordnance qualified person (UXOQP), qualified in accordance with Department of Defense Explosives Safety Board (DDESB) Technical Paper (TP)-18 (DDESB, 2020), and approved by 775 CES/CED, Explosive Ordnance Disposal (EOD) Squadron. Karen Wallner and Ethan Upton of UDEQ, and Karl Nieman of 75-Civil Engineering Group (CEG) accompanied the inspectors on April 15, 2024; and Chris Lear of 75-CEG accompanied the inspectors on April 15 and 16, 2024. All work was conducted in accordance with the UTTR-North Accident Prevention Plan (APP) (URS, 2022), and all SMP activities employed the unexploded ordnance (UXO) avoidance protocol described in the APP. A summary of inspection results is presented in Table 2-1. Site maps are provided in Appendix A, photologs are provided in Appendix B, and inspection forms are provided in Appendix C. The status of each institutional control at each of the 25 SWMUs representing 35 locations is discussed in the following sections. 2.2.1 Fencing The majority of UTTR-North is fenced and patrolled with guarded access points and surveillance to control unauthorized access to this military restricted area. No new site-specific fencing was required for the 25 SWMUs representing 35 locations, and inspection of the range boundary fencing is not included in the SMP. 2.2.2 Corner Markers According to the SMP, corner markers are recommended at all sites with suspected subsurface munitions debris except those adjacent to active targets and sites that are covered with concrete or asphalt. Corner markers were not placed at locations N004, N014, N015, N020, N021, N024, N027-1, N027-2, N030N, N030M, N030S, N062, N065, and N067 because buried waste is not suspected at these sites. In addition, corner markers were not placed at locations N037, N039N and N039W because they were observed in the field to be adjacent to active targets, and, therefore, either interfering with the site usage, or non-durable. Corner marker locations provided in Appendix D were recorded using a Magellan MobileMapper global positioning system (GPS). 2 Site Inspection Activities CHAPTERTWO Site Inspection Activities 2024 SMP Management Activity Report 2-2 RCRA SWMUs - UTTR North Projects\UTTR PBR\CLIN0044\ UTTR SATOC W9128F-23-F-0153 IRP_RCRA SWMUs_2024 SMP Activity Report.pdf Corner markers were set with 5/8-inch diameter rebar with a total length of 2 feet, set to within 6 inches of the ground surface, and topped with a marked aluminum cap. The markers are stamped with “No Dig Area”, the location number, and the corner number on the cap. The corner numbers begin with 1 at each location and are numbered consecutively. For example, at Location N048W where there are a total of four corner markers, the final cap reads “UTTR-North, N048W, 4 of 4, No Dig Area.” The corner markers installed during the initial SMP implementation meet the functional, visual, and durability objectives of the SMP, and follow standard surveying/corner setting practice. (The specific construction and GPS location accuracy of the corner markers deviates from that specified in the SMP. In lieu of one foot cubes of concrete with brass caps, 5/8-inch diameter rebar with stamped aluminum surveying caps were installed in accordance with the industry standard. This was done to mitigate safety risks of setting 150-pound concrete blocks, and to increase the visibility of the markers. Permanence and function is not affected. Recordation of distance and azimuth to permanent landmark features was not completed because of the predominant lack of permanent features at the majority of sites. Location was recorded by GPS methods in accordance with industry practice.) All corner markers were identified and observed to be in good condition during the 2024 inspection. 2.2.3 Signage Signage is recommended at all sites in the SMP. Inspections were conducted at every site to determine sign condition and adequacy. “Acceptable” signs were defined as being visible from the most likely access point; including a legible warning message, site identity, and contact information; and having the signpost (painted with Dakota Brown-colored paint) in good condition. Unacceptable or missing signs are repaired or replaced as needed. In 2024, sign maintenance was conducted at location N037W, N039N, and N092 as described in Table 2-1. All signs were deemed acceptable after the 2024 repairs were completed; however, the site number stickers on the N030M and N030S site signs are faded and it is recommended that the stickers be replaced next year. The initial inspection (2014) documented the current land use of each site as a baseline. In subsequent annual inspections the current year condition will be compared to the baseline condition to determine if unauthorized activity has occurred at a site. Photologs for the photos taken in 2024 are included in Appendix B. The photologs include photographs taken during the previous inspection year facing the same direction from the same point at each location to facilitate comparison. The site maps in Appendix A show the location and direction each photo was taken. In addition, AFCEC Environmental Restoration issues an annual memorandum to commanders, directors, staff directors, division chiefs, and tenants commanders regarding procedures to minimize exposure at contaminated sites. The memorandum provides background information, maps, and procedures for obtaining additional information. A copy of the April 2024 memorandum is included in Appendix E. During the inspection, the team encountered 42 soil piles that had been added to N002 (SWMU Number 2, Cluster Bomb Unit [CBU] Valley Disposal Trenches). The piles were dumped on the edge of the site, within the corner markers that indicate the approximate boundary of subsurface munitions debris. This is in violation of SMP land use controls which do not allow for soil disturbance and require site conditions/land use to remain unchanged, and in violation of posted warning signs which specify that dumping is not allowed. None of the piles were dumped into the disposal trenches. The soil piles were examined by the inspection team, along with the UXOQP. Wood and metal construction type debris was observed (i.e. rebar and lumber) in several piles, as well as indeterminant metal debris. The UXOQP also located a single 20-millimeter (mm) round on the surface of one pile. Following discovery of the soil piles, Hill AFB 75 CEG and eventually 75 Security Forces Squadron completed an investigation and found out the soil was excavated from a nearby weapons test site in the CBU Valley. The test site is used to detonate general purpose bombs for aging and surveillance purposes. The site has been in use for this purpose since 2018, with approximately 100 detonations occurring since that time. Single bombs are set up and detonated, with the detonations analyzed using multiple cameras and sensors. Once a detonation is complete, the crater is backfilled, and the area is graded. In 2023, the 582 Missile Maintenance Squadron (MMXS) determined the test pad needed a new base because of over-compaction due to repeated detonations. 582 MMXS requested 75 CEG dig out the pad and replace the soil with sand. 75 CEG agreed and completed this task during a time when no testing was taking place. This was the first time soil had been removed from the test pad, and there has never been any soil sampling associated with the 582 MMXS testing. The only explosives associated with the 582 MMXS bomb testing has been Tritonal and PBXN-109. All bomb testing conducted at the site results in high-order detonations initiated with the bomb’s fusing mechanism (unlike a disposal operation which is usually initiated from outside the munition’s casing). High-order detonations result in complete consumption of the munitions propellant. The 582 MMXS has not experienced a low-order detonation during the testing at this CHAPTERTWO Site Inspection Activities 2024 SMP Management Activity Report 2-4 RCRA SWMUs - UTTR North Projects\UTTR PBR\CLIN0044\ UTTR SATOC W9128F-23-F-0153 IRP_RCRA SWMUs_2024 SMP Activity Report.pdf site. The wood and metal seen in the soil piles are from stands and sacrificial equipment that are expended during detonations. The UXO in the form of a 20-mm round observed on the surface of one pile did not come from 582 MMXS bomb testing. There are 20-mm rounds (expended and unexpended) throughout the entire CBU Valley from testing which occurred in the 1990s. The 20-mm round was likely pulled up with subsurface soil when 75 CEG excavated the pad, and there could be more in the soil piles due to the expanse of the 1990s testing. AFCEC and Hill AFB are working with UDEQ on final disposition of the soil piles. The Air Force recommends leaving the soil piles in place at N002. Hill EOD confirmed there is a potential UXO risk with moving the piles. Moving them also doesn’t reduce risk in general since the entire CBU Valley has a potential for UXO presence (i.e. 20-mm rounds). Regardless of any chemical constituents, the piles could not be disposed of offsite (i.e. Grassy Mountain) without sifting the soil to remove potential UXO. The risk evaluated for N002 in the Revised RAR (AFCEC, 2023) concluded that the UXO sites cannot be adequately sampled for a chemical risk evaluation, nor would it be safe to remediate the sites, due to the presence of UXO. Therefore, these sites defaulted to site management. The RAR also noted that UXO at these sites could potentially be comingled with range and target debris, other metal and chemical materials used in testing and training at the range, and incidental munitions and explosives of concern (MEC) potentially intermingled with the debris. Potential chemicals could be metals, volatile and semi-volatile organic compounds, and/or munitions constituents. Based on this RAR decision, leaving the soil piles in place at N002 would not change the current risk evaluation. AFCEC and Hill AFB are working on additional preventative actions, such as ways to better educate UTTR and base personnel about our environmental restoration sites. Informational slides have been prepared to add to various UTTR and base briefings such as the UTTR Range Safety Brief. The disposition of the soil piles at N002 and final preventative actions will continue to be monitored and reported outside of this Management Activity Report. Site N004 was previously in a corrective action program with intent to achieve closure, and therefore groundwater sampling has not been conducted as part of the annual inspections as of this time. Groundwater sampling was conducted in October 2016 to support site closure and results were incorporated into a Revised Risk Assessment Report (RAR) which was finalized in May 2023 and approved by UDEQ in June 2023. It is recommended that the SMP be reevaluated and modified to reflect that groundwater sampling is not required at N004. Location N060 was sampled annually for five years (2014-2018) as required by the SMP (AFCEC, 2014b). The SMP requirement was based on the results of the RAR which identified concentrations of six contaminants in soil at location N060 exceeding United States Environmental Protection Agency (USEPA) soil screening levels (AFCEC, 2014a). Groundwater monitoring well NR-060-001 was installed at location N060 on August 27, 2014, and was sampled annually between 2014 and 2018. None of the chemical constituents analyzed in groundwater sampled from monitoring well NR-060-001 exceeded USEPA regional screening levels (RSLs) for protection of groundwater. Hill AFB submitted a recommendation for no further groundwater sampling at SWMU 60 to UDEQ on November 2, 2018 (AFCEC, 2018). UDEQ approved the recommendation to cease annual water sampling and to properly abandon the well on December 28, 2018 (UDEQ, 2018). Well abandonment was completed on March 12, 2020. Because a number of modifications and changes to the SMP have been identified, the main recommendation of this report is to reevaluate the SMP and secure government and UDEQ approval of the modified document through the review and approval process. 4.1 SITE MANAGEMENT The 25 SWMUs representing 35 locations requiring site management at UTTR-North have been inspected and conditions of each site compared to the institutional controls established at the outset of site management implementation have been recorded. Of the 25 SWMUs representing 35 locations requiring site management at UTTR-North, two recommendations resulting from the inspections were made in 2024; it is recommended that the faded site sign stickers at N030M and N030S be replaced during the 2025 inspections. In addition, it is recommended that the new soil piles observed at N002 be left in place since there are potential risks associated with moving the piles and leaving them in place would not change the current risk evaluation in the Revised RAR (AFCEC, 2023). The disposition of the soil piles and final preventative actions will continue to be monitored and reported outside of this Management Activity Report. Comments provided by Division of Waste Management and Radiation Control (DWMRC), UDEQ on January 15, 2016, noted Table 4-1 of the SMP does not have a requirement for land use restrictions at Site 21. Land use restrictions are required for all sites in the SMP. Therefore, Site 21 will be subject to land use restrictions until the typographical omission in the SMP can be corrected in a formal modification. Comments provided by DWMRC, UDEQ on January 15, 2016, noted Table 4-1 of the SMP shows corner markers required for locations N014, N015, N037, N039N, and N039W. Corner markers are not required at locations N014 and N015 because buried waste is not suspected at these sites. Corner markers were not placed at locations N037, N039N and N039W because they were observed in the field to be adjacent to active targets and are either interfering with the site usage or non-durable. Therefore, Table 4-1 of the SMP will be modified to show no corner marker requirements for sites N014, N015, N037, N039N, and N039W, at the next SMP modification. Comments provided by DWMRC, UDEQ on January 15, 2016, noted a discrepancy between the Executive Summary and Table 4-1 of the SMP regarding the number of sites requiring a fence. Therefore, it is recommended that the executive summary, Table 4-1, and Appendix A of the SMP be updated during the next modification of the SMP to show that fences are required and already in place at SWMUs 9, 15, and 21. 4.2 GROUNDWATER SAMPLING As discussed in Chapter 3, groundwater sampling at N060 is no longer required (UDEQ, 2018). Environmental Restoration, AFCEC/CZOM Hill Installation Support Section has developed the following information to prevent worker exposure to contaminated soil, prevent contaminated dispersal, and satisfy U.S. Environmental Protection Agency (EPA), Utah Department of Environmental Quality (UDEQ) and Nevada Division of Environmental Protection (NDEP) requirements. The Land Use Controls (LUCs) stipulated in each of the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) decision documents are also captured in the map(s) provided. 2. Information on known contamination sites has been consolidated onto maps to help those who may be involved in projects where excavation may take place (e.g. project and facility managers, planners, 75 CES personnel, 75 ABW/SC, etc.). This information will help individuals understand where contamination is located on the Base, at the Little Mountain Test Facility and at the Utah Test and Training Range. The maps will be updated as additional information on contaminated areas becomes available. 3. The maps are available on the Hill Air Force Base (AFB) GEOBASE Web Viewer (https://geobase.hill.af.mil/viewer/). On the Map Layers menu, select the “Environmental Regulated Areas.” Within the Environmental Regulated Areas category, there are three sub- categories, “Hill AFB Environmental Treatment Systems”, Hill AFB Regulated Sites”, and “UTTR Regulated Areas”. Each of these categories have several layers containing Environmental Regulated Areas of different types. 4. While the map identifies known contaminated areas, chemicals of concern may exist in other areas. Any excavation in an area of industrial activity may encounter contaminated soil. If an unusual odor or soil discoloration is observed during excavation or trenching, the Environmental Restoration Section should be contacted immediately at 777-6916. If explosives or ordnance contamination is encountered during excavation or trenching, contact the Explosives Ordnance Division at 777-5502. 5. The maps satisfy requirements found in Chapter 15 of Department of the Air Force Guidance Memorandum to Department of the Air Force Instruction 32-7020, Environmental Restoration Program (2 February 2023). 6.
