Re: Sanitary Survey
Corrective Action Plans
In response to the Defeciency Report received on 10/24/2024 which was the result
of the Sanitary Survey performed by Leslie Freeman on 9/25/2024, the Water Board of the
Bryant's Fork Summer Home association proposes these corrective actions to correct the
reported deficiencies:
1-Minor deficiency L014: Spring collection box not present.
Deficiency Statement:
Rule requires each spring collection area to be provided with at least one collection box
to permit spring inspection and testing. (no collection box).
Corrective Action Plan:
Our spring does have a collection box that allows for inspection and testing. The corrective
action for this deficiency will be to provide photographs and location of the box. Not sure
why this was overlooked or mis-communicated during the survey. The collection box is
there and we will provide evidence in order to close this deficiency.
2-Significant deficiency V009: Storage tank lid lacks a functioning gasket between the lid and frame.
Deficiency Statement:
Rule requires the access opening to a tank to be furnished with a gasket between the lid and
frame. The significant deficiency must be corrected within 120 days of notification or have
a corrective action plan approved by DDW. (need gasket on lid)
Corrective Action:
In response to this deficiency, we installed a rubber gasket on the lid to provide the
proper seal between the lid and the frame. See Figure 1 below. We submit that this will
address the deficiency and provides an adequate corrective action.

3-Significant deficiency VL01: Storage tank lacks an overflow.
Deficiency Statement:
Rule requires all storage tanks to be provided with an overflow that discharges at an
elevation between 12 and 24 invhes above the ground surface or the rim of the receiving
basin. This significant deficiency must be corrected within 120 days of notification or
have a corrective action plan approved by DDW. (No overflow at wet well used for storage)
Corrective Action Plan:
Our storage tank has an overflow that meets the stated requirements. We will provide
photographs of the overflow. Not sure why this was overlooked or mis-communicated
during the survey. The overflow has always been there and we will provide evidence
in order to close out this deficiency.
These are the corrective action plans that we submit in response to the stated deficiencies. Please
review and acknowledge acceptance or suggested revisions.
