Approximately 800 feet after driving over La Verkin Creek, turn left onto 740 North, then turn left on Main Street, follow Main Street north to the pit on the west side of new road, the road ended at the entrance to the La Verkin pit. SOURCE CONTACTS: Beau Stratton, Materials Division Head beau.stratton@interstaterock.com OPERATING STATUS: Operating normally at the time of inspection PROCESS DESCRIPTION: Interstate Rock Products operates an aggregate plant. Bank run is loaded by front end loader into the Jaw crusher, then conveyed to the screens, and then some material to the cone crusher then back to a screen. The sized material is conveyed from the screens to the appropriate size storage piles and is loaded onto haul trucks to be transported to various job sites. Emissions are controlled by water sprays on the conveyors, crushers, and screens. The haul road is watered by water truck. The storage piles are watered by a side cannon mounted on the water truck. APPLICABLE REGULATIONS: Approval Order (AO) DAQE-AN142350004-22, dated November 23, 2022 NSPS (Part 60) OOO: Standards of Performance for Nonmetallic Mineral Processing Plants, NSPS (Part 60) A: General Provisions, 2 SOURCE EVALUATION: Name of Permittee: Permitted Location: Interstate Rock Products - La Verkin Pit 42 South 850 West La Verkin Creek Bridge Hurricane, UT 84737 Washington County, UT 84737 SIC Code: 1442: (Construction Sand & Gravel) Section I: GENERAL PROVISIONS I.1 All definitions, terms, abbreviations, and references used in this AO conform to those used in the UAC R307 and 40 CFR. Unless noted otherwise, references cited in these AO conditions refer to those rules. [R307-101] I.2 The limits set forth in this AO shall not be exceeded without prior approval. [R307-401] I.3 Modifications to the equipment or processes approved by this AO that could affect the emissions covered by this AO must be reviewed and approved. [R307-401-1] I.4 All records referenced in this AO or in other applicable rules, which are required to be kept by the owner/operator, shall be made available to the Director or Director's representative upon request, and the records shall include the two-year period prior to the date of the request. Unless otherwise specified in this AO or in other applicable state and federal rules, records shall be kept for a minimum of two (2) years. [R307-401-8] I.5 At all times, including periods of startup, shutdown, and malfunction, owners and operators shall, to the extent practicable, maintain and operate any equipment approved under this AO, including associated air pollution control equipment, in a manner consistent with good air pollution control practice for minimizing emissions. Determination of whether acceptable operating and maintenance procedures are being used will be based on information available to the Director which may include, but is not limited to, monitoring results, opacity observations, review of operating and maintenance procedures, and inspection of the source. All maintenance performed on equipment authorized by this AO shall be recorded. [R307-401-4] I.6 The owner/operator shall comply with UAC R307-107. General Requirements: Breakdowns. [R307-107] I.7 The owner/operator shall comply with UAC R307-150 Series. Emission Inventories. [R307-150] I.8 The owner/operator shall submit documentation of the status of construction or modification to the Director within 18 months from the date of this AO. This AO may become invalid if construction is not commenced within 18 months from the date of this AO or if construction is discontinued for 18 months or more. To ensure proper credit when notifying the Director, send the documentation to the Director, attn.: NSR Section. [R307-401-8] Status: Out of Compliance. No unapproved equipment was observed at the time of inspection. The source's 2023 Emission Inventory has been submitted and can be found in the attachments section. The new equipment onsite has been installed and is operational, but no notification has been made about the operating status of the equipment. Source has been notified of this requirement for future AO modifications. No further action is currently recommended. 3 Section II: SPECIAL PROVISIONS II.A The approved installations shall consist of the following equipment: II.A.1 La Verkin Aggregate Pit II.A.2 One (1) Jaw Crusher Capacity: 300 tph Manufactured: 1990 NSPS Applicability: Subpart OOO II.A.3 Two (2) Cone Crushers (1 New) Capacity: 300 tph, each Manufactured: 2004, each NSPS Applicability: Subpart OOO II.A.4 One (1) Impact Crusher (New) Capacity: 300 tph Manufactured: 1996 NSPS Applicability: Subpart OOO II.A.5 Three (3) Screens (1 New) 1. Size: 2 Deck - 5' x 16' Manufactured: 1980 NSPS Applicability: None 2. Size: 3 Deck - 6' x 20' Manufactured: 1980 NSPS Applicability: None 3. Size: 3 Deck - 5 x 16' (New) Manufactured:1996 NSPS Applicability: Subpart OOO II.A.6 Miscellaneous Conveyors/Stackers Manufactured: 1996 or newer NSPS Applicability: Subpart OOO II.A.7 Miscellaneous Mobile Equipment Loaders, bulldozers, excavators, haul trucks, and water trucks II.A.8 Three (3) Emergency Generator Engines Fuel: Diesel 1. Rating: 591 kW (810 HP) Manufactured: 2013 2. Rating: 127 kW (170 HP) Manufactured: 1999 3. Rating: 20 kW (26.8 HP) Manufactured: 2016 NSPS Applicability: Subpart IIII MACT Applicability: Subpart ZZZZ Status: In Compliance. No unapproved equipment was observed at the time of inspection. As noted in the 2022 inspection, the source no longer owns and operates the three generators, listed as equipment number II.A.8. The source removed those as they operate on line power. 4 II.B Requirements and Limitations II.B.1 The La Verkin Aggregate Plant shall operate in accordance with the following II.B.1.a The owner/operator shall not produce more than 250,000 tons of processed aggregate per rolling 12-month period. [R307-401-8] II.B.1.a.1 The owner/operator shall: A. Determine production with belt scale records, scale house records, or bucket scale records B. Record production on a daily basis C. Use the production data to calculate a new rolling 12-month total by the 20th day of each month using data from the previous 12 months D. Keep the production records for all periods the plant is in operation. [R307-401-8] Status: In Compliance. The rolling 12-month total from August 2023 to July 2024, for aggregate production is as follows: 73,757.11 tons of aggregate produced Records of production are recorded daily by the scale house. Monthly records are recorded with new rolling totals calculated by the 20th of each month. See the attachments section for a copy of the aggregate production totals. II.B.1.b The owner/operator shall not conduct aggregate processing, offsite hauling, bulldozing, or loader operations for more than 2,600 hours each per rolling 12-month period. [R307-401-8] II.B.1.b.1 The owner/operator shall: A. Determine hours of operation by monitoring and maintaining an operations log. The hours of operation of each bulldozer shall be added together to determine the total hours of bulldozing. The hours of operation of each loader shall be added together to determine the total hours of loader operations. B. Record hours of operation each day C. Use the hours of operation to calculate a new rolling 12-month total by the 20th day of each month using data from the previous 12 months D. Keep hours of operation records for all periods the plant is in operation. [R307-401-8] II.B.1.c The owner/operator shall not conduct aggregate processing, offsite hauling, bulldozing, or loader operations before 7:00 am or after 7:00 pm each day. [R307-401-8] II.B.1.c.1 The owner/operator shall keep and maintain the following records of operation for all periods of aggregate processing operations: A. Time operations begin each day for aggregate processing operations, offsite hauling operations, each bulldozer, and each loader B. Time operations end each day for aggregate processing operations, offsite hauling operations, each bulldozer, and each loader 5 C. The total hours of operation each day for aggregate processing operations, offsite hauling operations, each bulldozer, and each loader. [R307-401-8] Status: In Compliance. Hours of operation are tracked daily in a log that shows the start and end times of operations for each day. The earliest start time from August 2023 to July 2024, was 7 AM and the latest end time was 4:30 PM. A copy of the hours of operation log can be found in the attachments section. II.B.1.d The owner/operator shall weigh and account for all aggregate material prior to the aggregate material leaving the site or being used in another process on site. [R307-401-8] Status: In Compliance. Aggregate produced onsite is weighed at the scale house before leaving the facility or before moving to another operation onsite. II.B.1.e The owner/operator shall not allow visible emissions from the following emission points to exceed the following opacity values: A. Crushers - 15% B. Screens - 10% C. Conveyor Transfer Points - 10% D. Conveyor Drop Points - 20%, and E. All other points - 20%. [40 CFR 60 Subpart OOO, R307-401-8] II.B.1.e.1 Opacity observations of emissions from stationary sources shall be conducted according to 40 CFR 60, Appendix A, Method 9. [R307-201-3] Status: In Compliance. No visible emissions exceeded limits within this AO at the time of inspection. See the attached VEO form for additional information. II.B.1.f The owner/operator shall install water sprays on each crusher, screen, conveyor transfer point, and conveyor drop point on site to control emissions. Water sprays shall operate as necessary to prevent visible emissions from exceeding the opacity limits listed in this AO. [R307-401-8] II.B.1.f.1 The owner/operator shall perform monthly periodic inspections to check that water is flowing to water sprays associated with each crusher, screen, and conveyor. If the owner/operator finds that water is not flowing properly during an inspection of the water sprays, the owner/operator shall initiate corrective action within 24 hours and complete corrective action as expediently as practical. [R307-401-8] Status: In Compliance. Water sprays are checked monthly on all applicable equipment. The checks are recorded in a log. See the attachments section for a copy of the log. II.B.1.g Records of the water spray inspections shall be maintained in a logbook for all periods when the plant is in operation. The records shall include the following items: A. Date the inspections were made B. Any corrective actions taken C. Control mechanism used if sprays are not operating. [R307-401-8] Status: In Compliance. Water sprays are checked monthly on all applicable equipment. The checks are recorded in a log. See the attachments section for a copy of the log. II.B.1.h The owner/operator shall conduct an initial performance test for each crusher, screen, and conveyor transfer point on site. Performance tests shall demonstrate compliance with the limitations specified in Table 3 to Subpart OOO. [40 CFR 60 Subpart OOO] 6 II.B.1.h.1 Initial performance tests for fugitive emissions limits shall be conducted according to 40 CFR 60.675(c). The owner/operator may use methods and procedures specified in 40 CFR 60.675(e) as alternatives to the reference methods and procedures specified in 40 CFR 60.675(c). [40 CFR 60 Subpart OOO] II.B.1.h.2 The owner/operator shall keep and maintain records of the initial performance test for each crusher, screen, and conveyor for the life of the equipment. [40 CFR 60 Subpart OOO, R307-401-8] Status: In Compliance. The initial VEOs for all equipment, including the new equipment, are maintained by the owner of the pit. A copy of the VEOs can be found in the attachment section. II.B.2 All Haul Roads and Fugitive Dust Sources on site shall be subject to the following II.B.2.a The owner/operator shall not allow visible emissions from haul roads and fugitive dust sources on site to exceed 20% opacity. [R307-205-4, R307-401-8] II.B.2.a.1 Opacity observations of fugitive dust from intermittent sources shall be conducted according to 40 CFR 60, Appendix A, Method 9; however, the requirement for observations to be made at 15-second intervals over a six-minute period shall not apply. The number of observations and the time period shall be determined by the length of the intermittent source. For fugitive dust generated by mobile sources, visible emissions shall be measured at the densest point of the plume but at a point not less than one-half vehicle length behind the vehicle and not less than one-half the height of the vehicle. [R307-205-4, R307-401-8] Status: In Compliance. No visible emissions from fugitive dust sources exceeded limits established within the AO. See the attached VEO form for additional information. II.B.2.b The owner/operator shall apply water to fugitive dust sources to prevent visible emissions from exceeding the opacity limits listed in this AO. The owner/operator may stop applying water to fugitive dust sources when the temperature is below freezing but shall apply other controls as necessary to prevent visible emissions from exceeding the opacity limits listed in this AO. [R307-401-8] II.B.2.b.1 The owner/operator shall keep records of water application and fugitive dust control for all periods when the plant is in operation. The records shall include the following items: A. Date and time treatments were made B. Number of treatments made and quantity of water applied C. Rainfall amount received, if any D. Records of temperature, if the temperature is below freezing E. Records of any other controls used to reduce fugitive dust. [R307-401-8] Status: In Compliance. Records of water application and sweeping are maintained and recorded. They are stored in a log for recordkeeping. See the attachments section for a copy of the waterlogs and sweeping logs. II.B.2.c The combined area occupied by all storage piles on site shall not exceed 3.0 acres. [R307-401-8] 7 II.B.2.c.1 To determine compliance with the total acres of the storage piles, the owner/operator shall measure the total area of the storage piles at least once every six months and shall maintain a record of the total acres of the storage piles. To determine the acres of the storage piles on site, the owner/operator shall use a handheld GPS unit and travel around the base of each storage pile on site to calculate the area of each storage pile on site. The area of each storage pile shall be added together to obtain the total area of all the storage piles on site. Records of the total acres of the storage piles shall contain the following: A. Date of measurements B. Size of each storage pile on site C. Total acres of all storage piles combined. [R307-401-8] Status: In Compliance. As of April 1, 2024, the storage piles onsite totaled 1.80 acres. Measurements are taken every 6 months in the spring and fall, using GPS technology. See the attachments section for a copy of the storage pile measurements. II.B.2.d The owner/operator shall not exceed 42.0 acres of active disturbed area. [R307-401-8] II.B.2.d.1 To determine compliance with the total acres of the active disturbed acreage, the owner/operator shall measure the total area of the active disturbed acreages at least once every six months and shall maintain a record of the total acres of the active disturbed acreages. To determine the acres of the active disturbed acreages on site, the owner/operator shall use Global Positioning System (GPS) measurements or aerial photographs. The area of each active disturbed acreage shall be added together to obtain the total area of all the active disturbed acreages on site. Reclaimed areas such as those with emerged vegetation or hydro-seeded do not contribute to the acreage totals. Records of the total acres of the active disturbed acreages shall contain the following: A. Date of measurements B. Size of active disturbed area on site C. Total acres of active disturbed acreage combined. [R307-401-8] Status: In Compliance. As of April 1, 2024, the disturbed acreage onsite totaled 18.11 acres. Measurements of disturbed acreage are calculated every 6 months in the spring and fall, using GPS. See the attachments section for additional information. II.B.3 The Unpaved Haul Roads on site shall be subject to the following II.B.3.a The owner/operator shall cover all unpaved haul roads and wheeled-vehicle operational areas with road base material, and an operational water truck shall be made available during each operating day. The owner/operator shall use water application to maintain opacity limits listed in this AO. If the temperature is below freezing, the owner/operator may stop applying water to the unpaved haul roads and wheeled-vehicle operational areas. The owner/operator shall resume applying water to the unpaved haul roads and wheeled-vehicle operational areas when the temperature is above freezing. [R307-401-8] 8 II.B.3.a.1 Records of water application shall be kept for all periods when the plant is in operation. The records shall include the following items: A. Date and time treatments were made B. Number of treatments made and quantity of water applied C. Rainfall amount received, if any D. Records of temperature, if the temperature is below freezing. [R307-401-8] Status: In Compliance. Water application records are maintained and recorded each time the water truck is used onsite. A copy of the logs can be found in the attachments section. II.B.4 The Paved Haul Roads on site shall be subject to the following II.B.4.a An operational vacuum sweeper and water truck shall be made available during each operating day. The owner/operator shall vacuum sweep and flush with water all the paved haul roads on site to maintain opacity limits listed in this AO. If the temperature is below freezing, the owner/operator shall continue to vacuum sweep the road but may stop flushing the paved haul roads with water. Flushing the paved haul road with water shall resume when the temperature is above freezing. If the haul roads are covered with snow or ice, the owner/operator may stop vacuum sweeping the paved haul roads. Vacuum sweeping the paved haul roads shall resume when the haul roads are cleared from snow and ice. [R307-401-8] II.B.4.a.1 Records of sweeping and water application on paved haul roads shall be kept for all periods when the plant is in operation. The records shall include the following items: A. Date and time treatments were made B. Number of treatments made and quantity of water applied C. Rainfall amount received, if any D. Records of temperature, if the temperature is below freezing E. Records shall note if the paved haul roads are covered with snow or ice. [R307-401-8] Status: In Compliance. Records of water application and sweeping are maintained and recorded. They are stored in a log for recordkeeping. See the attachments section for a copy of the water logs and sweeping logs. II.B.5 The Emergency Engines on site shall be subject to the following II.B.5.a The owner/operator shall not operate each emergency engine on site for more than 100 hours per rolling 12-month period during non-emergency situations. There is no time limit on the use of the engines during emergencies. [40 CFR 63 Subpart ZZZZ, R307-401-8] II.B.5.a.1 To determine compliance with a rolling 12-month total, the owner/operator shall calculate a new 12-month total by the 20th day of each month using data from the previous 12 months. Records documenting the operation of each emergency engine shall be kept in a log and shall include the following: A. The date the emergency engine was used B. The duration of operation in hours C. The reason for the emergency engine usage. [40 CFR 63 Subpart ZZZZ, R307-401-8] 9 II.B.5.a.2 To determine the duration of operation, the owner/operator shall install a non-resettable hour meter for each emergency engine. [40 CFR 63 Subpart ZZZZ, R307-401-8] Status: Not Applicable. The source no longer has the three generators onsite as the site operates on line power. II.B.5.b The owner/operator shall only use diesel fuel (e.g. fuel oil #1, #2, or diesel fuel oil additives) as fuel in each emergency engine. The owner/operator shall only combust diesel fuel that meets the definition of ultra-low sulfur diesel (ULSD), which has a sulfur content of 15 ppm or less. [R307-401-8] II.B.5.b.1 To demonstrate compliance with the ULSD fuel requirement, the owner/operator shall maintain records of diesel fuel purchase invoices or obtain certification of sulfur content from the diesel fuel supplier. The diesel fuel purchase invoices shall indicate that the diesel fuel meets the ULSD requirements, or the owner/operator shall obtain certification of sulfur content from the fuel supplier. [R307-401-8] Status: Not Applicable. The source no longer has the three generators onsite as the site operates on line power. The source has not purchased diesel fuel in the previous 12 months. Section III: APPLICABLE FEDERAL REQUIREMENTS In addition to the requirements of this AO, all applicable provisions of the following federal programs have been found to apply to this installation. This AO in no way releases the owner or operator from any liability for compliance with all other applicable federal, state, and local regulations including UAC R307. NSPS (Part 60) OOO: Standards of Performance for Nonmetallic Mineral Processing Plants Status: In Compliance. Source maintains all initial VEOs for applicable equipment onsite, including the new equipment. Water spray checks are conducted monthly and are stored in a log. A copy of the spray checks and the initial VEOs can be found in the attachments section. NSPS (Part 60) A: General Provisions Status: Compliance with Subpart A is determined by compliance with applicable federal subparts. In Compliance with Subpart OOO. AREA SOURCE RULES EVALUATION: The following Area Source Rules were evaluated during this inspection: Emission Standards: Fugitive Emissions and Fugitive Dust [R307-205] Status: In Compliance. No visible emissions from fugitive dust sources exceeded opacity limits at the time of inspection. Source maintains a water application log for each time watering is utilized to limit fugitive dust onsite. Stationary Sources [R307-210] Status: In Compliance. This state rule applies as the source is subject to NSPS Part 60 Subpart OOO. For compliance information, see Section III. 10 EMISSION INVENTORY: Listed below are the Actual Emissions Inventory provided from Interstate Rock Products - La Verkin Pit. A comparison of the estimated total potential emissions (PTE) on AO: DAQE-AN142350004-22, dated November 23, 2022, is provided. The 2023 Emission Inventory is listed in the box below. PTE are supplied for supplemental purposes only. COMPLIANCE STATUS & RECOMMENDATIONS: In regard to Approval Order (AO) DAQE-AN142350004-22, dated November 23, 2022, the overall status is: Out of Compliance with Condition I.8 of this AO. Source did not submit notification that the new equipment listed on this AO was operational within 18 months of the AO being active. Source was notified of this requirement for future AO modifications going forward. No further action is currently recommended. In compliance with the rest of the conditions listed within the AO. Records were made available via email upon request. Facility is well maintained. HPV STATUS: Not Applicable. RECOMMENDATION FOR NEXT INSPECTION: Inspect at a normal interval. NSR RECOMMENDATIONS: The generators on II.A.8 have not been on site since at least 2022. Evaluate for removal from the AO.
