1 PURPOSE AND SCOPE
1.1 Purpose
1.1.1 This work instruction is to document the outcome of the site-specific gap
analysis prior to the implementation of the Hearing Conservation
procedure. This gap analysis includes a review of all local, state, license,
and/or procedural requirements.
1.1.2 This work instruction is to document the process for evaluating noise
exposure.
1.1.3 This work instruction is to document the process for establishing and
maintaining an audiometric testing program.
1.1.4 This work instruction is to document the process for identifying and
labeling areas that require hearing protective devices to enter.
1.1.5 This work instruction is to document the process for conducting and
documenting training.
1.2 Scope
1.2.1 This covers the requirements in ES-SH-PR-305 for designated work
instructions CL-SH-WI-305-01, CL-SH-WI-305-02, CL-SH-WI-305-03,
CL-SH-WI-305-04, CL-SH-WI-305-05.

3 REQUIREMENTS AND GUIDANCE
3.1 Applicable Sections/Additional Requirements (01)
3.1.1 After a thorough procedure and regulatory requirement review, all sections
of ES-SH-PR-305, Hearing Conservation, are applicable and addressed in
this site-specific work instruction.
3.2 Process for Evaluating Noise Exposure (02)
3.2.1 The site/project shall administer a continuing, effective Hearing
Conservation Program (HCP) whenever an employee is exposed for at
least 1 day per calendar year to continuous or intermittent noise equal to or
exceeding an 8-hour time-weighted average (TWA) of 85 decibels on an
A-weighted (dBA), also known as the Action Level (AL) (OSHA criteria).

3.2.2 The site/project shall make all best-faith efforts to include for participation
in the site/project HCP each employee exposed to continuous or
intermittent noise for 1 day during a calendar year equal to or exceeding
an 8-hour time-weighted average (TWA) of 85 decibels on an A-weighted
scale (dBA), also known as the Action Level (AL) (OSHA criteria).
3.2.3 In order to determine areas and tasks that expose employees to levels of
noise at or above the AL, the Clive site will establish Similar Exposure
Groups (SEGs) related to noise exposure for personnel at the Clive site.
These SEGs shall be referenced in the Clive Industrial Hygiene Sampling
Plan (refer to the Clive site IH Sampling Plan) and may be adjusted as
required due to the changing nature of the work. As required,
representative sampling for the SEGs will be conducted so as to
reasonably assign risk, frequency or re-assessment, and protective
measures recommended/required for each SEG.
3.2.3.1 Examples of SEGs can include:
3.2.3.1.1 Workers conducting concrete chipping with hand
tools.
3.2.3.1.2 Workers conducting welding/grinding activities
during container fabrication.
3.2.3.1.3 Office workers/supervisors/managers who spend
less than 10% of their time in the field.
3.2.3.1.4 Equipment operators inside enclosed cabs, etc.
3.2.4 Noise evaluations conducted at the project may be informal, as part of
ongoing assessments of fieldwork activities, or formal as a part of the
Clive site’s IH sampling plan. Formal noise assessments shall utilize
related forms for noise sampling referenced in ES-SH-PR-305 or ES-SH-
PR-200.
3.2.5 Formal noise evaluations, as required, shall be conducted initially and
periodically and include Sound Level Surveys and Personal Noise
Dosimetry.
3.2.6 Sound Level Surveys
3.2.6.1 As required, a Sound Level Survey shall be conducted
initially to determine if hazardous levels of noise exist
within the Clive site. The Sound Level Survey shall be
designed and conducted/overseen by the Health and Safety

Department in order to identify employees potentially
exposed to levels of noise at or above the AL and enable
the implementation of engineering controls, and/or the
proper selection of hearing protectors, and the installation
of signage. Sound Level Surveys shall be documented
using a Sound Level Survey form (ES-SH-PR-305-F1 or
equivalent). Diagrams or “noise maps” showing the
location of each measurement taken should also be created.
3.2.7 Sound Level Surveys
3.2.7.1 As required, a Sound Level Survey shall be conducted
initially to determine if hazardous levels of noise exist
within the Clive site. The Sound Level Survey shall be
designed and conducted/overseen by the Health and Safety
Department in order to identify employees potentially
exposed to levels of noise at or above the AL and enable
the implementation of engineering controls and/or the
proper selection of hearing protectors, and the installation
of signage. Sound level surveys shall be documented using
a Sound Level Survey form (ES-SH-PR-305-F1 or
equivalent). Diagrams or “noise maps” showing the
location of each measurement taken should also be created.
3.2.8 Personal Noise Dosimetry
3.2.8.1 When appropriate, personal noise dosimetry can be
performed based off of the results of Sound Level Surveys
(formal or informal). Personal noise dosimetry shall be
documented using a Personal Noise Dosimetry form (ES-
SH-PR-305-F2 or equivalent).
3.2.9 Additional Requirements
3.2.9.1 Sound level meters and personal noise dosimeters used for
formal assessments shall be calibrated at the Clive site prior
to and at the completion of each sampling event to ensure
accuracy, and the calibration data shall be recorded.
3.2.9.2 Calibration equipment, sound level meters, and personal
noise dosimeters shall be calibrated annually to a primary
standard per the manufacturer’s requirements.
3.2.9.3 When taking formal measurements, sound level meters and

personal noise dosimeters shall be set to measure A-scale
with slow response. All continuous, intermittent, and
impulsive sound levels from 80 decibels to 120 decibels
shall be integrated into the noise measurements.
3.2.9.4 Monitoring shall be repeated whenever a change in
production, process, equipment, or controls increases noise
exposures to the extent that:
3.2.9.4.1 Additional employees may be exposed at or above
the Action Level (AL).
3.2.9.4.2 The noise reduction provided by hearing protectors
being used by employees may be rendered
inadequate.
3.2.9.5 Each employee exposed at or above an 8-hour TWA of
85dBA shall be notified of the monitoring results within 5
working days of receipt by the site. Affected employees or
their representatives shall be allowed to observe any noise
measurements. (Refer to ES-SH-PR-200, Industrial
Hygiene, for an example of the employee notification letter
for sampling results.)
3.2.10 Hearing and Calibration Documentation
3.2.10.1 The Health and Safety Department will be responsible for
maintaining noise level surveys, personal noise dosimetry,
and equipment calibration documentation. This
documentation will be stored in the Risk Management
folder or in OnBase.
3.3 Process for Establishing and Maintaining an Audiometric Testing Program

3.3.1 All employees in the Hearing Conservation Program will receive
audiograms initially and annually thereafter.
3.3.1.1 Employees may be requested to complete additional
audiograms based on results and exposures from previous
testing or work conditions.
3.3.2 The Clive facility will utilize CORE Health Networks to schedule

audiometric testing, advisement on follow up procedures,
recommendations, and recordkeeping to fulfill section 6 of ES-SH-PR-
305, Hearing Conservation.
3.3.2.1 All employees may request copies of all testing results
through the Health and Safety department.
3.4 Process for Identifying and Labeling Areas That Require Hearing Protective
Devices to Enter (04)
3.4.1 All potentially hazardous noise areas must be clearly identified by signs
located at their entrances or boundaries. The designation of hazardous
noise areas and equipment will be based on this criterion:
3.4.1.1 Any work area or equipment where the sound pressure
level is 85 dBA or above (continuous or intermittent) will
be considered noise hazardous.
3.5 Process for Conducting and Documenting Training (05)
3.5.1 Employees required to be in the Hearing Conservation Program shall
receive training both initially and annually thereafter. At a minimum, this
training shall include:
3.5.1.1 An overview of the site’s Hearing Conservation Program.
3.5.1.2 The effects noise may have on hearing.
3.5.1.3 The purpose of hearing protectors, advantages,
disadvantages, and noise reduction of various types, and
instructions on selection, fitting, use, and care.
3.5.1.4 The purpose of audiometric testing and an overview of the
test procedures.
3.5.2 All employees may request a copy of the Hearing Conservation Standard
from the Health and Safety department. Copies of this standard will also
be posted in main employee break areas.
3.5.3 Initial and annual training will be documented through the Learning
Management System (LMS).
