Dear Mr. Musterspaush:
The Closure Plan for the above-referenced facility, received by the Division of
Environmental Response and Remediation (DERR) on November 30, 2001, has been approved
subject to the noted modifications, if any. The DERR office and the local fire department must
be notified 72 hours before beginning closure activities. local agencies may have additional
requirements for closure or may charge inspection fees. The Salt Lake Valley Health
Department is no longer performing closure inspections. Please be sure the DERR office
(not Salt Lake Valtey Health Department) is contacted at least 72 hours before the closure
takes place.
The Closure Plan Approval is effective for a period of one year from the date of approval.
If closure does not take place within one year, submittal of a new Closure Plan will be required,
unless otherwise approved by the Executive Secretary (UST).
Any change to the approved Closure Plan must be submitted, in writing, and approved by
the Executive Secretary (UST) before implementation. If contamination is suspected or found
during closure activities, you must report it to the DERR at (801) 536-4100 within 24 hours of
discovery.
Enclosed is a copy of the Closure Notice form, which must be completed and submitted
to the Executive Secretary (UST) after the closure is performed. Please submit the
environmental and Unified Soil Classification sample analysis data and Chain of Custody forms
with the Closure Notice as soon as possible, but no later than 90 days after tank closure.

materials in direct contact with the environment. On-site stockpiling of contaminated soils may be required prior to

State Rules and Regulations. The compliance review, performed on312712001, indicates you have not
satisfied the requirements of the Act and are out of compliance. To achieve compliance you must do the
following:
1)Submit evidence that you are properly using your Pollulert DC l0 I Interstitial Monitoring System by

Dear Tank Owner or Operator
The Underground Storage Tank Act specifies that yearly registration fees and Petroleum Storage
Tank (PST) Fund fees must be received 60 days after the due date. If they are nor. the Certificate
of Compiiance and.ior coverage under the Fund automatically lapses.
Our records indicate that you have not paid your yearly registration fee and interest and./or your Fund
assessments and late penalties for the amount indicated on the attached invoice. To continue to
maintain a valid Certificate and/or Fund Coverage, the past due assessments must be paid by
September l, 1999. If the past due fees have not been received by September l, 1999, the
Certificate and./or Fund Coverage will automatically lapse.
In the event you allow your Certificate and./or Fund Coverage to Iapse, your tank(s) cannot receive
delivery ofproduct or be operated and costs of investigating and cleaning up releases are no longer
covered by the PST Fund. Renewal of a lapsed Certificate of Compliance would require additional
time and considerable expense on your part.
Please return your invoice and a copy of this speed with your payment. If you have questions

To continue to maintain a valid Certificate of Compliance, the facility must be in compliance with Federal and
State Rules and Regulations. An inspection of your facility on February 16,1996, and a review of our records
has shown that the following information is lacking:
tll Document that you have had a qualified cathodic protection tester test your cathodic protection
systems on your tank and lines within the last three years.
l2l Submit evidence that you are properly using your Pollulert CD101 Interstitial Monitoring System,
by:
(a ) Documenting that you maintain a log of monthly checks of the monitor and probe and that
all alarms, if any, are justified within 24 hours or called in to DERR as suspected releases,
by submitting a copy of the log.
(b) Documenting that the Interstitial Monitoring System is properly installed, calibrated and
maintained by submitting a copy of the installers installation and calibration documentation.
(c ) Documenting that the monitoring well is now secured to prevent unauthorized access.
The above information must be submitted to this office by 4116196.If it is not received by the date specified,

RE Non-compliance of underground storage tanks at SUPER FORD , Utah;
Facility I.D. #4001733_.
The Utah Underground Storage Tank (UST) Act provides that the Executive Secretary OST) may revoke
a Certificate of Compliance if it is determined that a facility is not in substantial compliance with all state
and federal UST statutes, rules and regulations.
At the request of the Executive Secretary (UST), the Division of Environmental Response and
Remediation (DERR) performed a compliance review for the underground storage tanks at the above
facility. The compliance review, performed on 20 JUNE 1994 , indicates you have not satisfied the
requirements of the Act and are out of compliance. To achieve compliance you must do the following:
1. Submit a copy of a current precision tank tightness test for tank(s) # 1 .
2. Submit a copy of a current precision line tightness test for the product lines associated with tank(s)

3. Submit evidence that an approved method of leak detection is being performed.

oil underground storage tank and associated piping at the above
location using the LEAK COI.IPUTER Systen. we have reviewed the data
produced in c-on junction with this test for purposes of verifying
lhe results and certifying the tank systen. The test v,Ias performed
in accordance with the test protocol , and therefore, satisfies aII
requirements for such testing as set forth by NFPA 329-87 and USEPA
40 cFR Part 280.

The test of the used oil tank system is considered sufficient for certificationpurposes since the water leveI in the backfill area is below the point ofhydrostatic equilibriun on any portion of the tank or piping.
SPECIAL CONSIDERATION
RE: 4030 Gallon Used Oil Tank
The average leak rate calculated during this test is only the predicted volumechange. The computer was used to measure temperature change and calculate a leakrate based on temperature change only. The actual volume change hras measured asa product level changre in inches in the fill riser itself. The level changeduring the last hour of data was a decrease of 5/8". Using Table 5G-1 of the
AcuTest manual, this change converts to a -.LzO GPH change, which matches the
conputer calculated change to within -.0L0 GPH. Therefore, this tank and systemis tight at 22rr above tank top.
