Dear Mr. Cahoon:
In December 2014, the Division of Environmental Response and Remediation (DERR)
requested a Corrective Action Plan (CAP) to address petroleum-impacted soils which remained in
the subsurface following tank removal and over-excavation activities at this site in 2011. To date,
Holiday Oil has not submitted a CAP or a CAP report. In order to return this release to a path toward
no further action (closeout), a review of the file was recently undertaken. The DERR project
manager has recommended additional work at this time. The DERR requests that a single soil boring
be advanced near the northwest corner of the fuel canopy, preferably under the canopy on the west
side of the northwest dispenser. The purpose of the boring is to document the upper and lower limits
of residual petroleum in soil, and to document if soil is considered clean, before reaching the level
of groundwater. If groundwater is encountered before soil is thought to be clean, then a monitoring
well should be installed in the boring for potential ongoing groundwater monitoring. If installed, an
initial groundwater sample should be collected from the completed monitoring well following
proper well development procedures.
All environmental samples must be collected by a Utah-Certified sampler following
sampling protocols as outlined in Utah Admin. Code R311-205. The report associated with the
sampling event must be submitted to our office within 90 days from the receipt of this letter.
Environmental samples must be analyzed for gasoline constituents using the methods indicated in
the Environmental Media Sampling Handbook (June 1, 2021) available on the DERR website.

Following receipt of your soil sampling/groundwater monitoring report, further review of
your file will lead to either no further action (closeout) of your site, or, additional work may be
necessary as determined by the Division Director. This additional work may consist of performing
additional investigation activities, conducting corrective or remedial action to cleanup to established
levels, implementing groundwater or other types of environmental monitoring to assess trends in
contaminant levels, performing a risk assessment to establish site-specific cleanup levels, or other
related activities.
