Miller, LLC 1050 West 200 North Logan, Utah 84321 Re:Subsurface Investigation/Groundwater Monitoring Requirements Aboveground Petroleum Storage Tank Release LW’s Truck Stop, located at 1020 West 200 North, Logan, Utah Facility Identification No. 0100611, Release Site MPA Dear Mr. Miller: On August 2, 2023, the Division of Environmental Response and Remediation (DERR) received a Groundwater Monitoring Report for the above-referenced site. After reviewing this information, the DERR project manager has recommended further investigation to determine the full extent of subsurface contamination. During the groundwater monitoring activities on July 15, 2023, groundwater samples were obtained indicating concentration above State-established applicable standards. Groundwater monitoring well MW-16 needs replacement. The groundwater contamination needs further delineation to the south. Additionally, at least three more groundwater monitoring events are required. Groundwater monitoring events should take place every nine months. The Division Director requires that additional samples be collected to determine the full vertical and horizontal extent and degree of any subsurface petroleum contamination. All environmental samples must be collected by a Utah certified groundwater and soil sampler following sampling protocol as outlined in Utah Admin. Code R311-205. A report outlining the results of the first phase of work must be submitted to the DERR within 90 days of your receipt of this letter. Follow-up groundwater monitoring reports must be submitted within 30 days of each subsequent sampling event.
