R315.5-3 NOTIFICATION AND EPA HAZARDOUS WASTE IDENTIFICATION NUMBERS:
lf you generate betweenl 00 kilograms-1000 kg. (approx. 220-2200 pounds) of hazardous waste per month (or
between 0-1 kg. of acutely hazardous waste), you are classified as a Sma!! Quantity Generator, and you must notify the
Executive Secretary of the Utah Solid and Hazardous Waste Control Board and obtain an EPA lD number (accomplished by
notifying the Utah Division of Solid and Hazardous Waste).
lf you are a small quantity generator you must obtain an EPA lD Number in order to offer hazardous waste for management to a
transporter or treatment, storage and disposal facility (TSDF) that has an EPA lD Number.

Only use approved manifest form (87OO-22 and 8700-22A) when offering hazardous wastes for transport to a TSDF or use
manifest of state that TSDF is located in if required by that state.

Notify the Utah State Department of Environmental Quality (801-5364123 or 1€00-5726400; 24 hrs) if more than 1 Kg of
acutely hazardous waste (listed in 40 CFR 261.33(e)) or 100 Kg of hazardous waste (or material which when spilled, becomes
hazardous waste) is spilled. Provide the appropriate information[(listed in R315-9(c)] in the report. Notiff the National
Response Center when applicable.

HAZARDOUS WASTE EMERGENCY PROCEDURES
In the event of a major hazardous waste spill, the following guidelines will be followed:
1. Take appropriate action to maintain the spill and to minimize any threat to
human health or environment. Items to be used for containing a spill are floor dry, which
is located on the west wall of the shop, and sorbent roll which is located above the Kar
Products.
2. Contact the emergency coordinator for Ross Equipment. The emergency
coordinator is Ron Jenserl Shop Foreman. Home phone #571-3835.
3. If the spill is more than 100 kg (approx. 160 lbs. or 25 gallons) of hazardous
waste, you must contact the Utah State Department of Environmental Quality at 801-536-
4123 or I -800-572- 6400, 24 las.
4. The hazardous waste must then be put into containers (eg. 55 gallon barrels).
This includes all floor dry, sorbent, and any other materials used to contain the spill. If
necessary, contact Safety Kleen at 801-975-0742.
5. All persons responsible for the sprll, or witnessing the sprll, must fill out a
detailed report and issue it to the emergency coordinator as soon as possible, or within
two (2) working days.

HAZARDOUS WASTE TRAINING
This is to inform you about the proper storage and disposal of hazardous waste that is generated
by the shop. Hazardous waste is defined as: any solid, liquid, or contained gaseous material that
you no longer use, and either throw away, recycle, or store until you have enough to dispose. A
waste that poses a threat to human health or the environment.
Typical hazardous waste that is generated by the shop at Ross Equipment Company are:
-engine oil -antifreeze
-transmission fluid/power steering fluid -gear oil
-hydraulic oil
-diesel fueVgasoline
-sludge
Each hazardous waste must be disposed of in the proper container. Each container is designate,fl
as to what will be stored in it, and there is a slgn above each container.
The engine oil, transmission fluid/power steering fluid, gear oil, hydraulic fluid, and diesel fuel will
be disposed of in the sump on the north wall of the shop. The sump will then pump the waste into
a 500 gallon container located outside at the northeast corner of the shop. Gasoline is to be
stored in a separate container. Used filters are to be drained in the sump, and then may be thrown
in the garbage.
The antifreeze will be stored in two (2) barels that are labeled "ANTIFREEZE. There is a sign
above the barrels designating them as "Antifreeze Only'. No other waste is to be put in these
barrels. The barrels are located outside of the shop on the east wall.
Any hazardous waste not listed previously is to be put in the barrel marked "Sludge". *Examples
of this waste include paint, safety solvent, or any unidentifiable substance.
*Please consult the shop foreman before placing any waste into this barrel. The barel is located
outside of the shop along the east wall. Some wastes may be reactive in the presence of other
wastes. This needs to be determined before they are combined.
The sump that is located on the south side of the shop catches all the water and waste that comes
from cleaning the shop floor. The sump is cleaned once a year by Safety Kleen. Do not pour any
hazardous waste down the surnp.
