Facility Identification No. 0100223, Release Site OLI
Dear Mr. Gurney:
The Division of Environmental Response and Remediation (DERR) received your Petroleum
Storage Tank Fund (PST Fund) Eligibility Application on October 15, 2024. The DERR has
determined that this release is qualified for full coverage from the PST Fund.
The PST Fund coverage for this release is $1,000,000.00 with a $10,000.00 deductible.
Reimbursement from the PST Fund will be authorized for costs above your deductible amount that
are determined to be reasonable, customary, and legitimate. All work plans and budgets must be pre-
approved by the DERR prior to initiating work in order to receive PST Fund reimbursement.
If it is determined that the contamination is from a release that occurred during a period for
which the underground storage tank (UST) system was not covered by the PST Fund, the release may
be ineligible or only partially eligible for payments from the PST Fund.
If any work is required in the future, all work associated with this release must be performed
by a Utah-Certified UST Consultant with a current PST Fund Statement-of-Qualifications on file with
the DERR. All work plans and budgets must be pre-approved by the DERR prior to initiating work
in order to receive PST Fund reimbursement.
