The Division of Water Quality (DWQ) likes to provide facilities with ample time to complete and submit the required forms well in advance of the expiration date to help facilitate an on-time permit renewal.Pursuant to Utah Administrative Code R317-8-3.1(4), all UPDES permittees must apply for reissuance of the permit not less than180 days prior to the expiration date of the existing permit. A copy of the DWQ UPDES application form can be found at https://deq.utah.gov/water-quality/updes-permitting-program. Please note that this form has changed since your last permit renewal. You must also complete the “Utah Antidegradation Review Form for Applicants” found at https://deq.utah.gov/water-quality/antidegradation-reviews-water-quality .Please complete and return all applicable forms to DWQ as soon as possible. Due to the complex nature of the receiving streams, DWQ would request that the completed forms be submitted by December 31, 2025, but no later than July 1, 2026, so that we can begin to process your permit for an on-time renewal.
