Transmitted herewith please find the final Closure Notification and laboratory analysis results for
samples collected during the UST removal activities completed recently at the former Maverik store
#199 located in Salt Lake City, Utah (FID #4001356). Six (6) closure soil and two (2) closure
groundwater samples were collected for analysis at the locations in Figure 1 in accordance with the
approved UST Closure Plan.
On December 19, 2023, two (2) existing steel tanks and two (2) dispenser islands and associated piping
were removed and disposed of off-site. A total of six (6) soil samples collected at various depths for
analysis at the locations shown on the attached site map (Figure 1). Groundwater was encounter at
approximately 9 feet below ground surface and therefore, two samples were collected. Samples were
analyzed at Chemtech Ford Analytical Laboratories in Sandy, UT, for methyl tertiary-butyl ether
(MTBE), benzene, toluene, ethylbenzene, xylenes, and naphthalene (MBTEXN), Total Petroleum
Hydrocarbons (TPH) – Gasoline Range Organics, and TPH - Diesel Range Organics.
Closure Soil Sampling
Closure soil samples were taken just above the water table on all 4 sides of the tank basin and directly
below the two removed dispensers. Samples were collected according to the appropriate EPA Method
protocols, into laboratory-supplied containers and placed on ice for transport to Chemtech Ford
Analytical Laboratories.
Soil samples were also field analyzed using a MiniRAE 3000+ portable VOC meter/photoionization
detector (PID), calibrated to 100 parts per million (ppm) isobutylene standard. Soil was placed into a
plastic ziplock bag, sealed, and warmed. The sealed bag was opened slightly to insert the tip of the VOC

meter to check for the presence of hydrocarbon vapors. PID readings were found to be low (less than
10 ppm) for all locations with the exception of SS-3 (under the northernmost dispenser) which was
slightly more at approximately 30 ppm.
Please see the attached site map (Figure 1) and for sampling locations.
Analytical Results
The laboratory analytical results for closure samples were all either non-detect or significantly below
initial screening levels for soil (Table 1, attached). Laboratory analytical results for groundwater (see
Table 2) identified benzene, toluene, xylenes, TPH-GRO and TPH-DRO above RBCA Tier 1 Screening
Levels in both GW-1 and GW-2 samples. Please note that Maverik has not sold or stored diesel fuel at this
location.
Analytical results along with the Chain of Custody forms are attached.
UST Disposal
The removed tanks were taken to Metro Group, Inc. in Salt Lake City for disposal.

During excavation of the tank basin, there were some musty smells along with some stained fill soil. PID
measurements, however, generally indicated very low to no hydrocarbon vapors. As such, overexcavation
was determined to be needed and not conducted. Likewise, there was no visual indication that there were any
impacts to the groundwater.
