Maverik, Inc. (Maverik) is pleased to provide results from the past two quarterly groundwater monitoring
events performed on July 26 and October 4, 2024, at the former Maverik Store #199 site in Salt Lake
City, Utah. Groundwater sampling and analysis results are discussed in this report.
GROUNDWATER SAMPLING AND ANALYSIS
Groundwater monitoring field activities were performed on July 26, 2024, and October 4, 2024, by
Maverik personnel according to Utah Division of Environmental Response and Remediation (DERR)
protocols. Mark Stewart, a Utah certified soil and groundwater sampler (GS-1842), gauged the
monitoring wells and collected groundwater samples for analysis. Groundwater samples were
collected from all five monitoring wells (MW-1, MW-2, MW-3, MW-4 and MW-5) during both sampling
events (see Figures 1 and 2).
Chemtech-Ford Analytical Laboratories of Sandy, Utah performed the groundwater analyses.
Samples were analyzed for benzene, toluene, ethylbenzene, total xylenes, naphthalene (BTEXN),
methyl tertiary-butyl ether (MTBE) according to EPA SW-846 Method 8260D, total petroleum

hydrocarbons – gasoline range organics (TPH-GRO) according to EPA SW-846 Method 8260B/C and
total petroleum hydrocarbons - diesel range organics (TPH-DRO) using EPA SW-846 Method 8015C.
Chain-of-Custody protocol was observed.
GROUNDWATER MONITORING RESULTS
Static Water Level Evaluation
The relative heights for the top of casing elevations and groundwater depth (based on laser level
surveys established over an arbitrary base point) were determined, as shown in Table 1. Groundwater
depths measured in July from the top of casing at each well ranged from 7.01 to 8.15 ft. and water
elevation, relative to the baseline, differed by up to 0.26 feet across the site and October measurements
differed by up to 0.25 feet across the site. Groundwater flow was determined to be in the
west/northwest direction during both events.
Groundwater Quality Evaluation
Groundwater analytical results associated with the wells are summarized in Table 2. Analytical reports
and Chain-of-Custody forms are attached. In July 2024, concentrations were above Utah Initial
Screening Levels (ISLs), but well below Utah Risk Based Tier 1 Screening Levels (RBCA Tier 1 SLs),
for: Total Petroleum Hydrocarbon – Diesel Range Organics (TPH-DRO) in MW-2 @ 2.3 mg/L and
benzene @ 0.007 mg/L in MW-3. In October 2024, concentrations were above ISLs, and again well
below RBCA Tier 1 SLs, in MW-2 for: TPH-Gasoline Range Organics (TPH-GRO) @ 2.3 mg/L, TPH-
DRO @ 2.9 mg/L, and benzene @ 0.009 mg/L. All other samples and constituents were either below
laboratory detection levels or at or below ISLs.
Please note that the well labeled MW-10 in the attached laboratory reports was a duplicate for MW-3,
in July, and MW-5, in October. The results for both duplicates correlated very well with the results of
the original samples. Also note that, although this is a groundwater monitoring report, for
completeness, I have included as Table 3 soil sampling results taken on May 14, 2024, in which all
constituents in all bores were at or below laboratory detection levels.

CONCLUSION AND FUTURE COURSE OF ACTION
In three rounds of sampling, the two mentioned previously in this report, and sampling conducted on
5/15/2024, all analytes were below laboratory detection levels in MW-1 (located just east and
upgradient of the former tank basin) and MW-4 and MW-5 (both of which are downgradient of the
former tank basin). All analytes were also below laboratory detection levels in MW-3 during the
October sampling event (which is upgradient of the tank basin). MW-2, which is located within the
former tank basin away from the property boundaries and utilities, had one or more analytes above
ISLs during all three sampling events.
Based on the findings presented in this report, that the analytical results during the last two quarterly
events were all significantly below RBCA Tier 1 SLs, and the few constituents that had results just
above ISLs are upgradient of the former tank basin or away from property boundaries and utilities,
Maverik respectfully requests that DERR consider issuance of a No Further Action letter for this site.
