Box 144840 195 North 1950 West, 1st Floor Salt Lake City, Utah 84114-4840 Subject: Closure Notice and Over-excavation Report Former Maverik Store #145 95 North Main St. Centerville, UT 84014 Facility ID #3000026 Release Site OLV Dear Mr. Harris: Transmitted herewith please find the final Closure Notification and laboratory analysis results for samples collected during the UST removal activities completed recently at the former Maverik store #145 located in Centerville, Utah (FID #3000026). Seven (7) closure soil and five (5) confirmation (over-excavation) samples were collected for analysis at the locations in Figure 1 in accordance with the approved UST Closure Plan. On October 2, 2024, two (2) existing steel tanks within a single tank basin and three (3) dispenser islands/underground containment sumps and associated piping were removed and disposed of off-site. The locations of the twelve total soil samples collected at various depths for analysis are shown on the attached site map (Figure 1). Groundwater was not encounter up to the maximum depth that was excavated (approximately 14 feet below ground surface (bgs)). All samples were analyzed at Chemtech Ford Analytical Laboratories in Sandy, UT, for methyl tertiary-butyl ether, benzene, toluene, ethylbenzene, xylenes, and naphthalene (MBTEXN), and Total Petroleum Hydrocarbons (TPH) – Gasoline Range Organics. Note that according to the USGS (https://waterdata.usgs.gov/), groundwater depth around the project area appears to be much greater than 50 feet bgs. Closure Soil Sampling and Soil Over-Excavation Activities Closure soil samples were taken on all four sides and at the base (approximately 12 ft bgs) of the tank basin and directly below the removed dispensers. Samples were collected according to the appropriate EPA Method protocols, into laboratory-supplied containers and placed on ice for transport to Chemtech Ford Analytical Laboratories. Mr. Gary Harris Former Maverik #145 UST Removal Report November 1, 2024 Page 2 Soil samples were also field analyzed using a MiniRAE 3000+ portable VOC meter/photoionization detector (PID), calibrated to 100 parts per million (ppm) isobutylene standard. Soil was placed into a plastic zip lock bag, sealed, and warmed. The sealed bag was opened slightly to insert the tip of the VOC meter to check for the presence of hydrocarbon vapors. PID readings taken at SS-7 (below the southernmost dispenser) identified notable vapor readings along with a very noticeable hydrocarbon smell and soil discoloration. As such, over-excavation activities around this location, was initiated. Along with following the obvious visual and olfactory evidence, periodic PID measurements were taken to guide the over-excavation activities. The excavation was slowly expanded until clean edges (as evident by relatively low PID readings) appeared to be reached. Confirmation soil samples were then collected at the north, south, west, and east excavation walls at approximately 8.5-9 feet below ground surface and from the bottom of the excavation at approximately 9 feet bgs. Please see the attached site map (Figure 1) and photographs of the over-excavation area. Analytical Results The laboratory analytical results for all closure samples and confirmation samples were all either non- detect or significantly below initial screening levels for soil (Table 1, attached). Confirmation samples collected along the bottom, north, and south of the overexcavation area (SS-7-Bottom, SS-7-North, and SS-7-South) were below laboratory detection levels for all analyzed constituents. There were detections for multiple constituents for the east and west confirmation soil samples (SS-7-East and SS-7-West) but, as mentioned before, all were below initial screening levels. Analytical results, please refer to Tables 1 and 2, along with the Chain of Custody forms are attached. Soil and UST Disposal A total of 65 tons of soil was over-excavated and disposed off-site at the E.T. Technologies – Soil Regeneration Site in Salt Lake City, UT (Non-Hazardous Waste Manifests attached). Clean imported fill materials were used to backfill the excavations. The removed steel tanks were taken to Metro Recycling in Salt Lake City for disposal/recycling. Findings and Conclusions Maverik identified petroleum impacted soil during tank removal and sampling activities in one location, under and around the southernmost dispenser. As such, overexcavation was conducted to remove the soil until clean edges were obtained. Confirmation soil sampling, after overexcavation, showed levels below Utah initial screening levels demonstrating that petroleum impacted soils were removed. Based on the anticipated depth to groundwater (>>50 ft bgs) and getting clean soil samples at the bottom of the over-excavated area, there are no impacts to groundwater. Maverik elected to over-excavate approximately 11.5' (length) x 11.5 ' (width) x 9' (depth) around this area remove the contaminated soil. The PID was used to find likely clean edges and the impacted soil was placed on a concreted area, covered until it was hauled to E.T. Technologies Soil Regeneration for disposal.
