Subject: NOTICE OF VIOLATION-TREATMENT TECHNIQUE VIOLATION, WATER SYSTEM UTAH29077
Dear Mr. CHzuSTIANSEN
Our records indicate that your system is in violation of the State of Utah Public Drinking Water Rules as follows:
Failure to address the following signifîcant deficiencies resulting in a Treatment Technique violation:
VL02 Storage structure missing proper air vent ST001
Utah Public Drinking Vy'ater Rules require all signifìcant dehciencies to be corrected within 120 days of the date of
discovery (usually the date of a sanitary survey) g the system must enter into a corrective action plan with the
Division of Drinking Water to address the signifrcant deficiencies as specified in R309-215-16(3)(a)(v). Once the
deficiency has been corrected you must notiff the Division of that correction within 30 days of completing the
correction. The system's failure to do so has resulted in a treatment technique violation in accordance with
R309-2 I s- I 6(4Xa).
As a result of this violation, the system must notifr all consumers within one year of the violation (see Sections R309-
215-16(3)(a)(vii) and R309-220-7 of the Rules). This public notice requirement cannot be waived once a violation
has occurred. You may provide the notice by mail to each customer, or by posting a notice, or in your Consumer
Confidence Report. A copy of the notice you send to each customer must be forwarded to us within one year of the
date of this letter. Please refer to the enclosed example of a public notice.
The system is advised to correct the significant deficiencies or enter into a corrective action plan as soon as possible.
If any of the above deficiencies have been comected please complete the enclosed IPS Deficiency Correction Notice
and submit the information to the Division of Drinking Water immediately. Please contact Jennifer Yee at 801-536-
