Recently the Division has learned that the design engineer is no longer available to fulfill this request. The Division understands that this project consisted of installing a number of water lines and fire hydrants. To the extent possible, these facilities were inspected during the Sanitary Survey that was conducted on September 18, 2024. The Division has determined that the installed improvements meet the requirements for operating permit issuance. On this basis, an Operating Permit for the Water Lines and Hydrants project is hereby issued as constituted by this letter. Please maintain a copy of this letter with your permanent records for future reference. If you have any questions regarding this Operating Permit, please contact Curtis Page, of this office, at (435) 650-7795, or Michael Newberry, Permitting and Engineering Manager, at (385) 515-1464.
