Subject:After-The-Fact Operating Permit, South Water Line Extension, and In-Line Booster Station (PF001); Eastland Special Service District, System #19013, File #06782
Dear Janet Ross:
During a plan review database audit, the Division of Drinking Water (the Division) discovered the system did not seek an Operating Permit for the subject project. The Division issued
Plan Approval for this project on April 28, 2006. No other submittals were received. Recently the Division has learned that the design engineer is no longer available to fulfill this
request.
The Division understands that this project consisted of extending an 8-inch diameter water main (DS001) approximately 5 miles to the south, adding an In-Line Booster Station (PF001),
adding a flow switch to an existing booster pump, and the addition of 16 residential service connections.
To the extent possible, these facilities were inspected during the Sanitary Survey that was conducted on September 18, 2024. The Division has determined that the installed improvements
meet the requirements for operating permit issuance. On this basis, an Operating Permit for the South Waterline Extension and In-Line Booster Station project is hereby issued as constituted
by this letter.
Disinfection Byproduct Sample Site Plan Updates
The water system must submit a revised disinfection byproduct (DBP) sample site plan due to the distribution system changing as a result of the water line extension [R309-210-10(43)(c)].
The system’s DBP sample site plan must include at least 1 site. A sample site plan template with instructions is enclosed with this letter. The system must submit an updated DBP sample
site plan by no later than December 31, 2024. Please contact Luke Treutel at Ltreutel@utah.gov or (385) 258-6084 with any questions regarding DBPs and DBP sample site plans. The completed
plan can be emailed to Luke or to DDWReports@utah.gov.
