Dear Mekeisia Westwood:The Division of Drinking Water (the Division) received a request for an Operating Permit for the 500k Tank from your consultant, Wyatt Payne with Ensign Engineering
on November 13, 2024. The Division issued Plan Approval for this project on April 5, 2023. DEQ Central District Engineer, John Chartier, conducted an onsite final inspection of this
facility on August 13, 2024.
Our understanding of the project is that a new 500,000-gallon concrete tank has been constructed near the two existing concrete tanks. The new tank is buried and set at the same elevation
as the existing tanks. Approximately 3,700 feet of new 12-inch diameter PVC pipe has been installed to connect the new tank to the distribution system. The new tank is known as the 500K
Tank and identified as ST003 in the Division’s database
We have received the following information for the Circleville Town 500K Tank (ST003):
Certification of Rule Conformance with plan approval conditions by a professional engineer
As-built or record drawings.
Satisfactory bacteriological results as evidence of proper disinfection and flushing.
Evidence of O&M manual delivery.
We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the 500K Tankis hereby issued as constituted by this letter.
You may now place the 500K Tank in service in your water system.
Please maintain a copy of this letter with your permanent records for future reference.
