HomeMy WebLinkAboutDDW-2024-013409 November 20, 2024 Dustin McCormick Maeser ID PO Box 90 Vernal, Utah 84078 Subject:Operating Permit, Water Line from Yellow Hill to Burdick Batch Plant; System #24004, File #13502 Dear Dustin McCormick:The Division of Drinking Water (the Division) received a request for an Operating Permit for the subject project from your engineering consultant, Horrocks, on November 18, 2024. The Division issued Plan Approval for the project on September 12, 2023. The Division understands the project consisted of constructing approximately 2 miles of 12-inch diameter HDPE water line to provide production water to the Burdick Material batch plant. The line is sized for fire flow and future domestic demand, and includes valves, fire hydrants and other appurtenances. The Division has received the following information for the project: Certification of Rule Conformance with plan approval conditions and changes made during construction were in conformance with rules R309-500 through 550 by a professional engineer. As-built drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. The Division has determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the water line from Yellow Hill to Burdick Batch Plantis hereby issued as constituted by this letter. You may now place the water line in service in your water system. Please maintain a copy of this letter with your permanent records for future reference.
