Subject:After-The-Fact Operating Permit, Well Discharge Piping Improvements

Dear Janet Ross:
During a plan review database audit, the Division of Drinking Water (the Division) discovered the system did not seek an Operating Permit for the subject project. The Division issued
Plan Approval for this project on October 16, 2001. No other submittals were received. Recently the Division has learned that the design engineer is no longer available to fulfill this
request.
The Division understands that this project consisted of improvements to the existing well head to meet Utah Drinking Water Rules.
To the extent possible, these facilities were inspected during the Sanitary Survey that was conducted on September 18, 2024. The Division has determined that the installed improvements
meet the requirements for operating permit issuance. On this basis, an Operating Permit for the Well Discharge Piping Improvements project is hereby issued as constituted by this letter.
