HomeMy WebLinkAboutDDW-2024-013406 November 20, 2024 Kevin Grant Barnes Church Wells SSD 1785 Wells Road Kanab, Utah 84741-8176 Subject:Operating Permit, Church Wells SSD 2023 Improvements Project, Water Line Replacement and 300 K Concrete Tank (ST002); Church Wells SSD, System #13002, File #12866, SRF #3F1824 Dear Kevin Barnes:The Division of Drinking Water (the Division) received a request for an Operating Permit for the Church Wells SSD 2023 Improvements Project from Kelly Chappell, P.E., Ensign Engineering, on November 13,2024. Previously, The Division received the plans and specifications on November 15, 2022. Written review comments were provided to your consultant on December 6, 2022. The Division received revised plans and specifications on January 26, 2023. Additional information was needed to evaluate the project. This additional information was provided to the Division on April 6 and April 18, 2023, The Division issued Plan Approval for this project on May 3, 2023. Our understanding of the project was to make several improvements to the Church Well SSD water system by replacing water lines and adding a new water storage tank. A booster pump station called Indian Hills Booster Station was originally included in the project, but due to funding constraints, it was not constructed. It was intended for growth in the upper pressure zone of the system, but since no homes have been constructed there, it was not needed at this time. Below is a more detailed description of each of these elements. Water Line Replacement – The water system replaced the 4-inch diameter water lines and much of the 6-inch diameter water lines that existed. This included the installation of 11,115 linear feet of 8-inch diameter PVC C-900 and 3,390 linear feet of 10-inch diameter PVC C-900 water line. It also included fire hydrants, isolation valves, service connections, and all appurtenant fittings. 300 K Concrete Storage Tank – This included the construction of a new 300,000-gallon reinforced concrete storage tank. The tank was constructed with all required elements including an inlet, outlet, drain and overflow piping; an access hatch; access ladder; and air venting. This facility is referred to as ST002 in the Division’s database. We have received the following information for the Church Wells SSD 2023 Improvements Project: Certification of Rule Conformance with plan approval conditions by a professional engineer. As-built or record drawings. Satisfactory bacteriological results as evidence of proper disinfection and flushing. Evidence of O&M manual delivery. We have determined that all conditions for operating permit issuance have been met. On this basis, an Operating Permit for the Church Wells SSD 2023 Improvements Project including water line replacement and a new 300K Tank (ST002) is hereby issued as constituted by this letter. You may now place these facilities in service in your water system. Please maintain a copy of this letter with your permanent records for future reference.
