EXECUTIVE SUMMARY
Underwood Environmental Consulting, Inc. conducted confirmation soil sampling and groundwater
monitoring at K & B Service in Lewiston, Utah in October 2024. The subsurface sampling
investigation was conducted to assess current subsurface conditions as requested by the DERR project
manager. The petroleum release was originally reported more than two decades ago. The project site
is currently operated as a gas station, convenience store, car wash, and automotive repair business.
The site is eligible for Petroleum Storage Tank (PST) Fund reimbursement.
This report summarizes the results of a subsurface sampling performed in October 2024. Based on
field observations and the analytical results of soil and groundwater samples, the following conclusions
are presented:
• Seven shallow exploratory soil borings were installed at the site. The borings were
installed at or near previous sampling locations.
• The native soil types encountered beneath fill included silty sand and sandy silt to the
depth explored (6 feet). Groundwater was encountered at about 5.5 feet below grade.
• Petroleum hydrocarbons were detected in two confirmation soil samples in excess of Tier
1 Screening Levels.
• Five confirmation soil sample locations had hydrocarbon concentrations less than
previously detected.
• Petroleum hydrocarbons were detected in one groundwater sample in excess of Initial
Screening Levels. All other groundwater samples collected and analyzed were either
below ISLs or non-detect.
• The groundwater flow direction was determined to be northeast.
• No measurable free product was observed in the monitoring wells.
The results of 2024 confirmation sampling at the project site are mixed. The groundwater analytical
results are generally encouraging with 8 of 9 well locations either being non-detect or below ISLs. The
soil analytical results, at least from the southeast portion of the property, are also encouraging. Given
that the southeast portion of the property appears to now be “clean” (CS-F and CS-G), UEC believes
the remedial system was effective.
However, the elevated hydrocarbon concentrations detected in soil at two locations (CS-A west of
water line and CS-D south of front door) are discouraging. It is uncertain whether the remedial system
was effective in those areas, or not, or whether hydrocarbons have migrated into those areas since the
remedial system was shut down.

1.0 INTRODUCTION
Underwood Environmental Consulting, Inc. (UEC) conducted confirmation soil sampling and
groundwater monitoring at K & B Service in Lewiston, Utah in October 2024. The subsurface
sampling investigation was conducted to assess current subsurface conditions as requested by the
DERR project manager (DERR, 2023). Soil samples were collected from the subsurface at locations
where elevated petroleum hydrocarbons had previously been detected. Groundwater samples were
also collected from all monitoring wells associated with the site. The DERR identifies the project site
as Facility ID #0100181, Release Site LQD. The site is eligible for Petroleum Storage Tank (PST)
Fund reimbursement and the work was conducted under approved work plan LQD-22.
This report summarizes the results of soil and groundwater sampling performed in October 2024 at the
site.
2.0 SITE DESCRIPTION
K & B Service is located at the intersection of Center Street and Main Street in Lewiston, Cache
County, UT (Figure 1). It is our understanding the project site was developed as a service station in
approximately 1979. An underground storage tank (UST) system and appurtenances was removed and
replaced in approximately 2002. Subsurface investigations including installation of monitoring wells
took place in 2003 and 2005. In 2009, an over-excavation of hydrocarbon impacted soil took place on
the south portion of the property. In 2010, the water line servicing the property was replaced. In 2015,
a remedial system utilizing soil vapor extraction (SVE) and oxygen diffusion was installed. The
remedial system ran for about three years until about 2018. An off-site monitor well (MW-11) was
installed in 2019.
A groundwater monitoring well network exists at the project site. Nine monitor wells, in various
condition, are present. Four of the monitoring wells are located on adjacent properties. Groundwater
monitoring of the project site dates back to 2003.
The adjacent properties include commercial and residential use. Groundcover consists of asphalt and
concrete pavement. The current UST fueling system consists of one compartmentalized gasoline and
diesel tank and a retail dispenser area. The dispenser area has an overhead canopy. The project site is
flat and is at an elevation of approximately 4,508 feet above mean sea level.
3.0 METHODOLOGY
An “additional work” letter from the DERR, dated December 21, 2023, specified soil sampling
locations to be revisited for confirmation. The locations requested had previous elevated benzene or

other hydrocarbons detected. The prior sampling locations requested, with dates, and the resultant
confirmation sampling IDs are as follows.

3.1 SOIL BORINGS
Seven locations were scrutinized using ground penetrating radar (GPR) to clear for utilities and other
shallow subsurface features. The soil borings were installed on October 14, 2024 using direct-push
drilling methods. Some borings were “daylighted”, others were not. Soil samples were collected in a
continuous fashion to the total depth of each boring as drilling progressed. Groundwater was observed
at about 6 feet below grade. Once soil samples were obtained from the target depth at each location,
the borings were backfilled with bentonite and patched at the surface match existing.
The subsurface logs are provided in Appendix A.
3.2 SOIL SAMPLING
Soil samples were collected from 5-foot acetate liners provided by the driller. Soil samples were
collected from each boring location and retained in laboratory-supplied glass jars, labeled
appropriately, and placed on ice. The soil samples were analyzed by Chemtech-Ford Laboratories for
methyl tertiary-butyl ether (MTBE), benzene, toluene, ethylbenzene, xylenes, naphthalene (BTEXN),
total petroleum hydrocarbons - gasoline range organics (TPH GRO), and TPH - diesel range organics
(TPH DRO) using EPA methods 8260B/C, 8260D, and 8015C. Chain of custody documentation was
maintained for all samples from the time of collection until delivered to the laboratory.
Field screening of soil sample intervals was conducted by headspace analysis using a photo-ionization
detector (PID). The samples were field screened using a Mini RAE 3000 PID. The exploratory
borings were logged in the field by a UEC professional geologist using the unified soil classification
(USC) system.

3.3 GROUNDWATER SAMPLING
On October 10, 2024, water levels were measured in nine existing monitoring wells using an electronic
water level indicator. A three-casing purge volume of water was removed from each well using either
a peristaltic pump or a disposable bailer. The groundwater samples were labeled appropriately, placed
on ice, and submitted to the laboratory under chain-of-custody. The groundwater samples were
analyzed for MTBE, BTEXN, TPH GRO, and TPH DRO using EPA methods 8260B/C, 8260D, and
8015C.
4.0 RESULTS
4.1 SUBSURFACE LITHOLOGY
Beneath a concrete or asphalt surface, the borings encountered primarily silty sand (SM) or sandy silt
(ML) to the depth explored (6 feet). Groundwater was observed at about 5.5 feet below grade.
Hydrocarbon staining and/or odor were observed at four locations (CS-C, CS-D, CS-E, and CS-F) in
narrow zones associated with the capillary fringe or water table.
The subsurface logs are provided as an attachment in Appendix A.
4.2 FIELD SCREENING
Field screening of soil was conducted at the 5 to 5.5 foot interval at each boring location using a PID.
The results of field screening are presented on the subsurface logs attached in Appendix A. The field
screening generally suggested that hydrocarbon impacts were greatest in soil near the water table. The
highest PID readings were observed at CS-A and CS-F.
4.3 SOIL ANALYTICAL RESULTS
One soil sample was submitted for laboratory analysis from each boring location. A summary of soil
sampling results for Release LQD is presented on Table 2. The analytical results presented on Table 2
(and Table 3) are compared with Tier 1 Screening Criteria (Tier 1 levels) and Initial Screening Levels
(ISLs) (DERR, 2005).
Elevated hydrocarbon concentrations were detected in soil samples at two boring locations (CS-A and
CS-D). Tier 1 levels were exceeded for total xylenes and TPH-GRO in soil sample CS-A @ 5’. Tier 1
levels were exceeded for benzene, total xylenes and TPH-GRO in soil sample CS-D @ 5’.
Hydrocarbon concentrations detected in soil sample CS-B @ 5’ were above ISLs, but, were less than
previous soil sample SS-3 (Table 3).

Low hydrocarbon concentrations were reported for soil samples at two boring locations (CS-C and CS-
E). No hydrocarbons were detected in soil samples collected from borings CS-F and CS-G (Table 2;
Table 3; Figure 1).

Results in bold print exceed Tier 1 levels
A review of the comparison table above indicates positive results at sample locations CS-B, CS-C, CS-
E, CS-F, and CS-G. The hydrocarbon concentrations detected at locations CS-A and CS-D, however,
appear similar or greater than previously detected at those locations.
The laboratory analytical reports and chain-of-custody documentation are provided in Appendix B.
4.4 GROUNDWATER FLOW DIRECTION
In October 2024, the depth to groundwater ranged from approximately 5.1 to 6.3 feet below top-of-
casing (TOC) in the nine monitoring wells (Table 2). Figure 2 depicts a site map and a groundwater
contour for the project site on October 10, 2024. Based on the calculated groundwater elevations, the
groundwater flow direction is to the northeast.
4.5 GROUNDWATER ANALYTICAL RESULTS
Nine monitoring wells were purged and sampled on October 10-11, 2024. A summary of groundwater
monitoring data is presented on Table 4. The analytical results presented on Table 4 are compared
with Tier 1 levels and ISLs (DERR, 2005).

Hydrocarbon concentrations in groundwater exceeded ISLs at MW-1. The highest benzene
concentration detected was 0.163 mg/L and the highest TPH concentration detected was 5.2 mg/L
TPH-DRO. Low hydrocarbon concentrations, below ISLs, were detected in groundwater at MW-5 and
MW-11. No hydrocarbons were detected in the groundwater samples collected from six wells (MW-2,
MW-6, MW-7, MW-8, MW-9 and MW-10) (Table 4; Figure 2).
The laboratory analytical reports and chain-of-custody documentation are provided in the Appendix B.
5.0 QUALITY ASSURANCE AND QUALITY CONTROL
Soil and groundwater samples collected during subsurface investigation were collected by a State-
certified soil and groundwater sampler under the supervision of a State-certified UST Consultant.
Proper sampling protocol, utilizing industry recognized standards, was followed during the
investigation. Samples were retained in laboratory-supplied containers (4 oz. glass jars or 40 ml vials)
and placed on ice until relinquished to the laboratory. Chain of custody records were maintained and
are attached along with the complete laboratory analytical reports in Appendix B. The analytical
laboratory utilized is certified by the State for the analyses performed and has quality assurance /
quality control procedures in place to ensure the accuracy of the analytical data provided.
6.0 CONCLUSIONS AND RECOMMENDATIONS
Based on field observations and the analytical results of soil and groundwater samples obtained during
subsurface investigation, the following conclusions are presented:

7.0 LIMITATIONS
Our services consist of professional opinions from a State-certified UST Consultant, made in general
accordance with locally accepted environmental practices at the time of execution. Environmental
samples are obtained from specific locations, utilizing methodologies consistent with the Utah
Department of Environmental Quality regulations. Even when field investigative work is performed to
an appropriate standard of care, certain site conditions may present difficult assessment problems.
Such conditions may include, but are not limited to, complex geologic setting and materials, fate and
transport character of petroleum hydrocarbons, physical constraints, presence of utilities overhead and
below ground, and the limitations of the assessment tools employed. UEC makes no guarantee; either
expressed or implied, that the analytical results accurately represent subsurface conditions throughout
the entire project site.
